研究目的：

通过前瞻性研究，在驱动压（ΔP）指导下进行PEEP调节，比较不同通气模式（PCV-VG与VCV）对高体重指数（BMI≥40kg/m2）患者减重手术中呼吸力学（ΔP、Pplat、Ppeak、Cdyn）及氧合指数、肺部并发症（PPCs）的影响，以期可以实现术中ΔP降低，改善呼吸动力学指标，减少低氧及PPCs发生率。  

Objectives of Study：

This prospective study is aimed to investigate the impact of driving pressure (ΔP)-guided PEEP titration on respiratory mechanics (ΔP, Pplat, Ppeak, Cdyn), oxygenation index, and postoperative pulmonary complications (PPCs) in bariatric surgery patients with a body mass index (BMI) ≥40 kg/m2, comparing pressure-controlled volume-guaranteed (PCV-VG) and volume-controlled ventilation (VCV). The primary objective is to optimize intraoperative ΔP reduction, enhance respiratory dynamics, and mitigate hypoxemia and PPC incidence.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.择期行减重手术；
2.年龄18岁以上；
3.体重指数（BMI）≥40kg/m2；

Inclusion criteria

1.Selective for laparoscopic bariatric surgery; 2.Age over 18 years; 3.BMI>=40kg/m2;

排除标准：

1.术前动脉血氧分压＜45 mmHg， 2.PASP＞50 mmHg， 3.控制不佳的高血压、心衰等， 4.肺叶切除手术史， 5.术前两周内有感冒， 6.拒绝参加。

Exclusion criteria：

1. Preoperative arterial partial pressure of oxygen < 45 mmHg, 2. PASP > 50 mmHg, 3. Poorly controlled hypertension, heart failure, etc. 4. History of lobectomy, 5. Catching a cold within two weeks before surgery, 6. Refuse to participate.
