研究目的：

1.构建基于主动健康理念的老年HF患者智能居家容量管理模式，以期为开展老年HF患者居家容量评估和管理提供方便、有效的模式参考。 2.应用基于主动健康理念的老年HF患者智能居家容量管理模式，提高老年HF患者的心功能状态、自我管理能力、生活质量及自我效能感，降低心力衰竭躯体感知、非计划再入院率。  

Objectives of Study：

1. Establish an intelligent home capacity management mode for elderly HF patients based on the concept of active health, in order to provide a convenient and effective mode reference for the home capacity assessment and management of elderly HF patients. 2.The intelligent home capacity management mode of elderly HF patients based on the concept of active health is applied to improve the cardiac function status, self-management ability, quality of life and self-efficacy of elderly HF patients, and reduce the physical perception and unplanned readmission rate of heart failure.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄＞=60岁； 2.诊断符合《中国心力衰竭诊断和治疗指南2018》中心力衰竭的诊断标准； 3.符合美国纽约心脏病学会心功能分级标准Ⅱ至Ⅲ级； 4.知情同意； 5.无精神疾患，无认知障碍，愿意正常沟通交流。

Inclusion criteria

1. age >= 60 years; 2. diagnosis in accordance with the diagnostic criteria for heart failure in the Chinese Guidelines for the Diagnosis and Treatment of Heart Failure 2018; 3. compliance with the American New York Heart Association cardiac function grading standard grade II to III; 4. informed consent; 5. no mental illness, no cognitive impairment, willing to communicate normally.

排除标准：

1.急性心力衰竭发作期； 2.意识不清，存在语言、认知障碍等不能配合； 3.心力衰竭晚期； 4.合并其他系统严重疾病。

Exclusion criteria：

1. acute heart failure episodes; 2. unconsciousness and the presence of speech and cognitive deficits that prevent cooperation; 3. Advanced stage of heart failure; 4. Combination of other systematic serious diseases.
