研究目的：

1. 通过文献回顾、专家小组讨论结合设备使用说明，制定MII-pH监测技术的标准化护理操作流程及精准护理干预方案； 2. 应用MII-pH监测技术于机械通气患者，探讨该技术在降低机械通气患者误吸发生率中的应用效果。  

Objectives of Study：

1. through literature review, expert group discussion and equipment instructions, formulate standardized nursing operation process and precise nursing intervention scheme of mii-ph monitoring technology; 2. apply mii-ph monitoring technology to patients with mechanical ventilation, and explore the application effect of this technology in reducing the incidence of aspiration in patients with mechanical ventilation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥18 岁；（2）应用成人 ICU 患者误吸风险评估量表得分≥37分者；（3）接受机械通气≥48 h；（4）患者或家属知情同意。

Inclusion criteria

(1) Age >= 18 years old; (2) The score of aspiration risk assessment scale for adult ICU patients was >= 37; (3) Receiving mechanical ventilation for >= 48 hours; (4) informed consent of patients or family members.

排除标准：

（1）插管前已有误吸和（或）呼吸机相关性肺炎；（2）实施俯卧位通气；（3）外院转入不明气管插管时间；（4）有食管静脉曲张、食管损害或（和）梗阻、食管狭窄患者。

Exclusion criteria：

(1) Aspiration and/or ventilator-associated pneumonia had occurred before intubation; (2) Implement prone position ventilation; (3) The time of tracheal intubation was unknown; (4) Patients with esophageal varices, esophageal lesions or (and) obstruction, and esophageal stenosis.

机械通气48h以上且胸部X线或CT 显示新出现或进展性的浸润影、实变影或磨玻璃影，加上下列 3 种临床症状中的 2 种或以上，可建立临床诊断：发热、体温＞38.0℃; 脓性气道分泌物；外周血 WBC＞10×109/L 或 4×109/L和(或核左移)。呼吸机相关性肺炎发生率=呼吸机相关性肺炎发生例数/同期机械通气患者例数×100%。

用SPSS 26.0统计软件产生 144 个随机数字（数量暂定）,将其按从小到大的顺序排列，前 72 个数字为 A 组，后 72 个数字为 B 组，随后重新生成两个随机数字分别代表 A 组和 B 组，设定试验组为较大值数字，对照组为较小值数字。

Randomization Procedure (please state who generates the random number sequence and by what method)：

144 random numbers (tentative) were generated by SPSS 26.0 statistical software and arranged in descending order. The first 72 numbers were group A and the last 72 numbers were group B. then two random numbers were generated to represent group A and group B respectively. The experimental group was set as the larger number and the control group as the smaller number.
