研究目的：

本研究引入了MTM居家药学服务模式，研究该模式对老年肺结核合并冠心病患者社会支持、服药依从性、治疗效果、患者自我管理行为、药学服务满意度的影响，通过制定出合适老年肺结核合并冠心病患者的居家药学服务策略，以提高其社会支持、治疗依从性及自我管理能力，从而提高治疗效果，最终改善患者的临床预后，提升药学服务质量及满意度，具有良好的社会效益和经济效益，具有广阔应用前景，为居家药学服务在其他慢病患者中的应用提供参考。  

Objectives of Study：

In this study, the MTM home pharmaceutical care model was introduced to study the effects of this model on social support, medication compliance, treatment effect, patient self-management behavior and pharmaceutical care satisfaction of elderly patients with pulmonary tuberculosis complicated with coronary heart disease. By formulating a suitable home pharmaceutical care strategy for elderly patients with pulmonary tuberculosis complicated with coronary heart disease, it can improve their social support, treatment compliance and self-management ability, so as to improve the treatment effect, ultimately improve the clinical prognosis of patients, and improve the quality and satisfaction of pharmaceutical care. It has good social and economic benefits and broad application prospects. To provide reference for the application of home pharmaceutical care in other patients with chronic diseases.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1 患者年龄﹥60岁；
2 确诊为肺结核合并冠心病患者，资料基本完整，病情相对稳定；
3 出院后进行居家治疗者，患者和家属知晓研究内容，能配合可进行正常交流并自主完成问卷调查。
4 自愿参与本研究，并签署知情同意书。

Inclusion criteria

1 Patients ' age > 60 years old ; 2The patient was diagnosed with pulmonary tuberculosis complicated with coronary heart disease. The data were basically complete and the condition was relatively stable. 3 After discharge, patients and their families know the research content, can cooperate with the normal communication and complete the questionnaire survey independently. 4 Volunteer to participate in this study and sign informed consent.

排除标准：

1 耐药肺结核病患者、肺外结核患者；
2 无法配合本研究顺利完成的患者；
3 合并有心、肝、肾、脑等器官严重损害或恶性肿瘤疾病患者；
4 伴有严重免疫功能缺陷的患者；伴有严重精神类疾病者。

Exclusion criteria：

1 Patients with drug-resistant tuberculosis, patients with extrapulmonary tuberculosis ; 2 Patients who could not cooperate with the successful completion of this study ; 3 Patients with severe damage to heart, liver, kidney, brain and other organs or maligna;nt tumor diseases ; 4 Patients with severe immune dysfunction ; with severe mental illness.
