研究目的：

作为牵头单位联合四川省其他 14 家医疗机构开展极/超早产儿生后“黄金1小时 ”持续质量改进（quality improvement,QI）。在患儿生后 60min 内从以下七个方面进行管理措施的改进：1）产前咨询和团队组建；2）胎盘输血；3）体温管理；4）呼吸支持；5）脐血管理；6）循环管理；7）感染预防，具体管理措施 形成《早产儿管理手册》，由参与单位参照执行。收集改进前后患儿的临床资料并比较患儿结局，了解质量改进成效，及时修定临床措施，保障改进效果的实现。  

Objectives of Study：

As the leading institution, we collaborated with 14 other medical facilities in Sichuan Province to implement continuous quality improvement (QI) for the "Golden Hour" after birth in extremely/very preterm infants. Within 60 minutes after delivery, management measures were enhanced across the following seven aspects: 1) prenatal consultation and team formation; 2) placental blood transfusion; 3) temperature management; 4) respiratory support; 5) umbilical cord blood management; 6) circulation management; and 7) infection prevention. Specific management protocols were compiled into the "Preterm Infant Management Manual," which was adopted by all participating institutions for reference and implementation. Clinical data of infants before and after the improvement were collected and compared to assess patient outcomes, evaluate the effectiveness of the quality improvement initiatives, and facilitate timely revisions of clinical measures to ensure the achievement of improved outcomes.
