研究目的：

本研究的首要目的是比较两种肺癌筛查策略在65岁及以上的不同风险层级老年人群中的肺癌检出率差异。第一种策略是先普筛后分层，即在年度体检中直接向所有65岁及以上的老年人提供LDCT检查，无需事先进行高危人群识别；第二种策略是先分层后筛查，即先通过问卷评估识别高危人群，仅对高危人群提供LDCT筛查。 本研究的次要目的一是评估两种筛查模式的筛查效率与成本效益，分析不同风险人群在两种模式下的获益差异；二是比较不同筹资模式下的成本效益。  

Objectives of Study：

This study aims to compare the effectiveness of two lung cancer screening strategies among elderly individuals aged 65 years and above with different risk levels. The first strategy is a universal-first approach, in which all eligible older adults receive LDCT during their annual health check-ups without prior risk assessment. The second strategy is a risk-stratified approach, where high-risk individuals are first identified through a questionnaire and only those classified as high risk are offered LDCT screening. The secondary objectives are: (1) to assess the screening efficiency and cost-effectiveness of the two strategies and examine the differential benefits across risk groups; and (2) to compare the cost-effectiveness of the strategies under different financing models.

纳入标准：

1.两组研究区域管辖范围中常住居民 2.年龄 ≥ 65岁 3.参与基本公共卫生服务免费体检 4.江都组：2024年度体检选择进行CT升级 东台组：经问卷评估为肺癌高危人群，并同意接受LDCT筛查者。 5.既往无已确诊的肺癌病史

Inclusion criteria

1. Permanent residents within the jurisdictions of the two study areas. 2. Age >= 65 years. 3. Participation in the free annual health check-up under the Basic Public Health Service program. 4. Jiangdu group: Individuals who chose CT upgrading during the 2024 health check-up. Dongtai group: Individuals identified as high-risk for lung cancer through questionnaire assessment and who consented to undergo LDCT screening. 5. No prior history of confirmed lung cancer.

排除标准：

1.患有其他恶性肿瘤且已进入终末期治疗者 2.不愿意或无法配合完成问卷者 3.有明显CT检查禁忌证（如严重过敏史或呼吸不能配合屏气）者

Exclusion criteria：

1. Presence of other malignant tumors and currently receiving end-stage treatment. 2. Unwilling or unable to complete the questionnaire. 3. Clear contraindications for CT examination (e.g., severe allergy history or inability to cooperate with breath-holding during scanning).
