研究目的：

通过MCE联合rPMS进行为期4周的周期性干预分析CLBP人群干预前后不同平衡任务下的姿势控制水平改变情况；其次，深入探究MCE干预后肌肉与皮质激活表现。通过sEMG分析与姿势控制相关的深层和浅层肌肉以及下肢肌肉在运动中的激活状态以及肌肉共收缩表现，并借助fNIRS研究姿势控制相关脑区，如DLPFC, S1, M1, SMA在姿势控制中的活动情况。同时，着重探究大脑网络特征，明确皮质与皮质之间的连接关系，旨在为揭示MCE干预的作用机制提供功能连接证据，为临床诊断和康复治疗提供科学依据。  

Objectives of Study：

Through a 4-week cyclic intervention using MCE combined with rPMS, this study analyzed changes in postural control levels under different balance tasks in individuals with CLBP before and after the intervention. Furthermore, it delved into muscular and cortical activation manifestations following MCE intervention. Using sEMG, the study examined the activation states of deep and superficial muscles related to postural control, as well as lower limb muscles during movement, along with muscle co-contraction patterns. Additionally, fNIRS was employed to investigate the activity of brain regions associated with postural control, such as the DLPFC, S1, M1, and SMA, during postural tasks. The study also placed emphasis on exploring brain network characteristics to clarify connectivity between cortical regions, aiming to provide functional connectivity evidence for elucidating the mechanism of MCE intervention and to offer a scientific basis for clinical diagnosis and rehabilitation therapy.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.腰痛超过3个月，且近1月内未接受腰部治疗（运动疗法或非运动疗法如药物治疗、物理治疗等）； 2.年龄为18-30岁非体育专项普通大学生； 3.BMI：18.5-23.9Kg/m^2； 4.实验前一周内腰部疼痛视觉模拟评分：VAS超过3分但不高于7分，中等疼痛程度； 5.实验前一周内Oswestry功能障碍指数ODI：评分＞12%，中等功能障碍； 6.无脊柱、下肢手术及外伤病史； 7.无运动测试禁忌症（如严重高血压、心脏疾病、外周血管疾病、呼吸系统疾病等）。

Inclusion criteria

1. Low back pain for more than 3 months, and no low back treatment (exercise therapy or non-exercise therapy such as medication, physical therapy, etc.) in the past 1 month; 2. The age of 18-30 years old non-sports major ordinary college students; 3.BMI: 18.5-23.9Kg/m^2; 4. Visual analogue scale (VAS) of lumbar pain within one week before the experiment: VAS more than 3 points but no more than 7 points, moderate pain; 5. Oswestry disability index (ODI) : score > 12%, moderate dysfunction; 6. No history of spine and lower limb surgery or trauma; 7. No contraindications to exercise testing (such as severe hypertension, heart disease, peripheral vascular disease, respiratory disease, etc.).

排除标准：

1.椎管狭窄/骨折、神经根病或手术史； 2.身体臀部和大腿部有疼痛；下肢感觉、反射异常；除腰部外其他关节出现疼痛； 3.由特定的病理学因素导致的腰痛，如肿瘤、骨质疏松、腰椎骨折、风湿性关节炎、强制性脊柱炎、腰椎间盘突出伴有神经根受累、腰椎滑脱等； 4.并发肝、肾、造血系统、内分泌系统等严重原发性疾病； 5.并发皮肤疾病者； 6.孕妇及哺乳期妇女；精神病受试者； 7.试验前三个月内未服用药物（如止痛药等）。

Exclusion criteria：

1. History of spinal stenosis/fracture, radiculopathy, or surgery; 2. Pain in the buttocks and thighs; Abnormal sensation and reflex of lower limbs; Pain in joints other than the waist; 3. Low back pain caused by specific pathological factors, such as tumor, osteoporosis, lumbar fracture, rheumatoid arthritis, ankylosing spondylitis, lumbar disc herniation with nerve root involvement, lumbar spondylolisthesis, etc. 4. Serious primary diseases of liver, kidney, hematopoietic system, endocrine system, etc. 5. Concurrent skin diseases; 6. Pregnant and lactating women; Psychiatric subjects; 7. No medications (e.g., painkillers) taken within three months before the trial.

盲法：

本研究对测试评估者设置了盲法。测试评估者只负责数据的收集与分析，不参与本研究的其他环节。所有的测试评估者对受试者的病情、分组以及个人信息都是不知晓的，只对其提供试验受试者的编号、身高、体重以及病程等必要信息。为了更好降低测试评估者在测试过程中可能带来的主观偏倚，测试时由受试者在规定时间段内自主选择合适的时间进行测试，增加了两组受试者参与测试评估的随机性。

Blinding：

This study was blinded to the test assessor. The test assessors were only responsible for data collection and analysis and were not involved in any other part of this study. All test evaluators were unaware of the condition, grouping, and personal information of the subjects, and were only provided with the necessary information such as the number, height, weight, and disease duration of the test subjects. In order to minimize the subjective bias of the test evaluators during the test, the test was conducted at a time chosen by the subjects within a specified period of time, which increased the randomness of the participation of subjects in two groups in the test evaluation.
