研究目的：

1.分析结直肠癌造口患者出院计划需求，整合肠造口患者临床护理服务最佳证据。 2.基于罗伊适应模式,结合临床最佳证据及患者需求,构建结直肠癌造口患者出院准备计划。 3.验证结直肠癌造口患者出院准备计划的临床应用效果。  

Objectives of Study：

1. Analyze the discharge planning needs of patients with colorectal cancer ostomy, and integrate the best evidence for clinical nursing services for patients with colostomy. 2. Based on Roy's Adaptation Model, combined with clinical best evidence and patient needs, construct a discharge preparation plan for patients with colorectal cancer ostomy. 3. Verify the clinical application effect of the discharge preparation plan for patients with colorectal cancer ostomy.

药物成份或治疗方案详述：

1.证据综合 系统检索 近5年期间发表的肠造口患者护理服务相关文献,以整合临床最佳证据并对证据进行分级和推荐强度划分,最终形成肠造口患者护理服务最佳证据总结。 2.干预方案构建 基于罗伊适应模式结合肠造口患者护理服务最佳证据总结及患者需求分析,构建结直肠癌造口患者出院准备计划初稿,经专家函询形成干预方案终稿。 3.实证研究 选取我院普通外科符合纳入排除标准的患者作为研究对象，观察组患者采用基于罗伊适应模式的结直肠癌造口患者出院准备计划方案进行临床干预，对照组患者常规术后护理，比较出院时两组患者出院准备度、健康照护参与度、出院指导质量得分情况。以及出院时、术后第 4 周、术后第 12 周时肠造口相关并发症发生情况。 4.随访患者反馈 采用现象学研究方法,对结直肠癌造口患者进行半结构深入访谈,探讨结直肠癌造口患者出院计划的体验及感受，优化干预流程。 

Description for medicine or protocol of treatment in detail：

1. Evidence Synthesis: Systematically search for relevant literature on intestinal stoma patient care services published in the past 5 years to integrate clinical best evidence and classify and categorize the evidence, ultimately forming a summary of the best evidence for intestinal stoma patient care services. 2. Intervention Plan Construction: Based on Roy's Adaptation Model combined with the summary of the best evidence for intestinal stoma patient care services and the analysis of patient needs, draft the initial version of the discharge preparation plan for patients with colorectal cancer stoma. After expert consultation, the final intervention plan is formed. 3. Empirical Research: Select patients from the general surgery department of our hospital who meet the inclusion and exclusion criteria as the research subjects. The control group patients receive routine care for the pre-experiment, while the observation group patients receive clinical intervention based on the discharge preparation plan for patients with colorectal cancer stoma according to Roy's Adaptation Model. The control group patients receive routine postoperative care. Compare the discharge preparation degree, health care participation degree, and quality scores of discharge guidance of the two groups of patients at discharge, as well as the occurrence of intestinal stoma-related complications at discharge, the 4th week after surgery, and the 12th week after surgery. 4. Follow-up Patient Feedback: Using the phenomenological research method, conduct semi-structured in-depth interviews with patients with colorectal cancer stoma to explore the experience and feelings of patients with colorectal cancer stoma regarding their discharge plans, and optimize the intervention process. 

纳入标准：

1.年龄18-90 岁； 2.经病理诊断为原发性 CRC，且拟行造口手术； 3.具备阅读、理解和表达能力； 4.自愿参与并签署知情同意书。

Inclusion criteria

1. Age: 18 - 90 years old; 2. Diagnosed as primary CRC by pathology and scheduled for ostomy surgery; 3. Capable of reading, understanding and expressing thoughts; 4. Voluntarily participate and sign the informed consent form.

排除标准：

1.严重精神疾病或有精神系统疾病的家族史； 2.有严重合并症，如脑卒中、其他晚期恶性肿瘤、心力衰竭，严重肝肾功能不全等疾病的患者； 3.伴有肿瘤转移至其他部位。

Exclusion criteria：

1. Severe mental illness or a family history of mental disorders; 2. Patients with severe comorbidities, such as stroke, other advanced malignant tumors, heart failure, severe liver and kidney dysfunction, etc.; 3. With tumor metastasis to other parts.
