Objectives of Study: |
Locking plate osteosynthesis is one of the most widely used methods for fracture fixation. However, the nonstandard placement of locking head screws remains a concern. The present study aims to introduce a new classification and describe the epidemiology of the pseudolock phenomenon in upper limb and low limb locking plate osteosynthesis. |

纳入标准: |
1.成人病例(≥18周岁) 2.急性创伤性骨折病例 (<3周) 3.局限于四肢长骨的骨折(上肢:锁骨、肱骨、桡骨、尺骨;下肢:股骨、胫骨、腓骨) 4.使用带专用锁定孔的固定角度锁定钢板 5.接骨板和螺钉由钛合金或不锈钢制成 6.使用预塑形的接骨板 7.锁定头螺钉的直径在2.4毫米至6.5毫米之间 8.术后平片可以判断螺钉锁定状态 |

Inclusion criteria |
1. Adult patients (aged 18 years or older); 2. Acute traumatic fractures with an injury-to-surgery interval of less than 3 weeks; 3. Fractures limited to long bones in the extremities(lupper limb:clavicle, humerus, ulna, and radius. lower limb:femur, tibia, and fibula); 4. Utilization of fixed-angle locking plates featuring dedicated locking holes; 5. Constructs manufactured from titanium alloy or medical-grade stainless steel; 6. Preshaped plates designed to conform to anatomic templates; 7. Locking head screws with diameters ranging from 2.4 to 6.5 mm; 8. Postoperative orthogonal radiographs (anteroposterior and lateral projections) were utilized to evaluate the locking state of screws. |

排除标准: |
1.18岁以下患者 2.非急性骨折(手术间隔损伤超过3周)、肿瘤引起的病理性骨折、假体周围骨折或影响骨形态的先天性畸形(如成骨不全、软骨发育不全) 3.涉及指骨、掌骨、肩胛骨、盆骨、跟骨、距骨、跖骨、趾骨的骨折 4.使用非轴向载荷设计的远端锁定螺钉、皮质螺钉或松质骨螺钉(完全或部分螺纹) 5.使用没有专用锁孔的锁定板(例如,限触的动态压缩板) 6.使用可变角度锁定板,或者桥接系统 7.术后成像不足或影像资料缺失,无法评估螺钉锁定状态 8.锁定头螺钉直径小于2.4毫米或大于6.5毫米 9.使用非金属结构(如碳纤维、聚醚醚酮材料) 10.使用三维打印板或定制的锁定板 |

Exclusion criteria: |
1. Patients under 18 years of age; 2. Non-acute fractures (injury-to-surgery interval exceeding 3 weeks), pathological fractures due to neoplastic processes, periprosthetic fractures, or congenital malformations affecting bone morphology (e.g., osteogenesis imperfecta, achondroplasia) ; 3. Fractures involving the finger bones, metacarpals, scapula, pelvic ring, talus, calcaneus, metatarsals, or phalanges; 4. Use of distal locking screws with non-axial loading designs, as well as cortical or cancellous screws with full or partial threads; 5. Plates lacking dedicated locking holes (limited-contact dynamic compression plates); 6. Variable-angle locking plates (VA-LCP) or ortho-bridge system (OBS); 7. Non-metallic locking constructs (polyetheretherketone/carbon fiber composites); 8. 3D-printed or customized plates; 9. LHS outside of the 2.4–6.5 mm diameter range; 10. Inadequate postoperative imaging failing to evaluate the locking state of screws. |
