研究目的:

本研究旨在探讨球类游戏对自闭症儿童社会性注意影响的研究。具体而言,本研究选取轻中度自闭症儿童为实验对象,基于ICF-CY框架制定适合ASD儿童的球类游戏,通过10周的球类运动干预,结合眼动追踪技术中的自由观看范式和中央线索范式对社会性注意进行量化,并测量唾液催产素,分析球类运动对自闭症儿童社会性注意以及催产素水平的影响。  

Objectives of Study:

This study aims to investigate the effects of ball games on social attention in children with autism spectrum disorder (ASD). Specifically, the research focuses on children with mild to moderate ASD. Based on the ICF-CY framework, tailored ball game interventions were designed for ASD participants. Over a 10-week ball game intervention period, social attention was quantified using eye-tracking technology, including free-viewing paradigms and central cueing paradigms. Additionally, salivary oxytocin levels were measured to analyze the impact of ball game interventions on both social attention and oxytocin levels in children with ASD.

纳入标准:

(1)9-13岁的自闭症儿童;(2)对实验对象采用CARS量表进行自闭症评定:30分以下被视为无自闭症;30-37分为中度自闭症;37-60分为重度自闭症。选取轻中度的自闭症儿童(3)被试均通过机构与家长同意与被试的监护人签订知情同意书;(4)肢体健全,右利手,且手部活动不受限

Inclusion criteria

(1) Children with autism spectrum disorder (ASD) aged 9–13 years; (2) The Childhood Autism Rating Scale (CARS) was used to assess autism severity: Scores <30: Non-autistic (excluded) Scores 30–37: Mild-to-moderate ASD (included) Scores 37–60: Severe ASD (excluded) Only participants with mild-to-moderate ASD (CARS 30–37) were selected. (3) All participants were enrolled with institutional approval and written informed consent from their legal guardians; (4) Inclusion criteria:Physically intact limbs;Right-handedness;Unrestricted hand mobil

排除标准:

(1)因身体脏器有严重疾病而不适合运动的学生;(2)身体发育异常而不能进行运动者;(3)因情绪异常而不能配合完成测试者;(4)对唤其姓名没有反应或无法接受主试引领;(5)经临床医生确诊,共患精神病性障碍或癫痫;(6)短期内吃过对中枢神经系统有影响的药物

Exclusion criteria:

(1) Students with severe systemic/organ diseases contraindicated for physical exercise; (2) Individuals with physical developmental abnormalities preventing safe participation; (3) Those with behavioral/emotional dysregulation impairing test compliance; (4) Non-responsive to name-calling or unable to follow experimenter's guidance; (5) Clinically diagnosed comorbid psychotic disorders or epilepsy; (6) Recent use of centrally-acting medications (within protocol-specified timeframe).

基于 ICF-CY的球类游戏干预方案进行训练。具体如下 1运动类型 球类游戏干预方案以《ACSM 运动测试与运动处方指南(第十一版)》中推荐自闭症儿童的运动处方进行设计,该指南[50]明确表明:各种类型的身体活动已被证明对自闭症人群有益。对自闭症人群推荐的运动方式,分为 A、B、C、D 四种运动类型,其中 A 类是需要最少技能或体适能的耐力活动,B类运动是需要最少技能的较大强度耐力运动,C类运动是需要技能的耐力运动,D 类运动是休闲运动。结合其他指南推荐的运动方式主要分为有氧运动、增强肌肉力量活动、强化骨骼活动 3 类。因此,本研究康复方案采用 B 与 C结合类。球类游戏设计遵循儿童发展理论,首先使用ICF-CY自闭症儿童运动功能评估量表评估运动功能,然后根据ICF-CY结构,从身体结构与功能、参与与环境出发,结合运动康复目标(体适能类、基本动作技能类、游戏互动类),制定适合ASD儿童的球类游戏。同时自闭症儿童与其他儿童相比较敏感,为制定科学有效的实验教学计划,笔者通过与机构的相关老师、儿童的家长相沟通了解每个自闭儿童的喜好与特点,并与相关专家进行商讨制定。 2运动时间及频率 根据 2020 年发布的《WHO关于身体活动和久坐行为的指南》 ,建议残疾儿童和青少年应每天进行≥60min 中到高强度的运动,以有氧运动为主;每周应进行≥3d 中高强度有氧运动以及增强肌肉和骨骼的运动,此外,每周还应进行≥3d 增强功能平衡和力量训练的身体活动。基于此,本研究设计的球类干预方案为 3 次/周,45min/次,包括10分钟的热身活动,5分钟的整理放松活动,30分钟的球类体育游戏,进行10周,包括适应阶段,基础阶段,提升阶段,巩固阶段。 3运动强度 根据《ACSM 运动测试与运动处方指南(第十一版)》,自闭症儿童和成人的运动频率、强度和时间的设置应逐渐发展到一般人群的水平。同时基于文献的分析并结合受试者具体体能情况,本研究设置的运动强度为中等强度,通过计算个体心率负荷为:(220-年龄)×60%~70%=126~147。运动实施过程中,对被试儿童挑选3-5个佩戴polarV800手表进行全程监控,监测被试儿童在运动干预过程中的心率变化情况,保障运动强度达到中等水平,如心率过高或超出最大心率,则立即降低运动强度。心率测试获得的数据包括:被试儿童的平均心率,个人最低、平均和最高心率及其百分比等。

Ball Game Intervention Protocol Based on ICF-CY Framework 1. Exercise Type The ball game intervention protocol was designed according to the ACSM's Guidelines for Exercise Testing and Prescription (11th Edition), which explicitly states that various types of physical activity have proven beneficial for individuals with autism spectrum disorder (ASD) [50]. The recommended exercise modalities for ASD populations are categorized into four types (A, B, C, D):Type A: Low-skill/low-fitness endurance activities;Type B: Low-skill moderate-to-vigorous endurance exercises;Type C: Skill-dependent endurance exercises;Type D: Recreational activities .Additional guidelines classify exercise into three primary categories: aerobic exercise, muscle-strengthening activities, and bone-strengthening activities. This study adopted a combined B+C approach.The ball games were developed based on:Child development theory. First ,we use the ICF-CY Motor Function Assessment Scale for ASD children (evaluating motor proficiency),and then follow the ICF-CY framework (addressing Body Functions & Structures, Activities & Participation, and Environmental Factors) and rehabilitation objectives (fitness, fundamental movement skills, and interactive play) to ensure individualized adaptation, consultations were conducted with:Institutional teachers ,parents (to identify children's preferences and characteristics) and domain experts. 2. Duration & Frequency Following the 2020 WHO Guidelines on Physical Activity and Sedentary Behavior for disabled children/adolescents: Daily: ≥60 mins of moderate-to-vigorous aerobic activity Weekly: ≥3 days of aerobic + muscle/bone-strengthening exercises Additional: ≥3 days of functional balance/strength training Our protocol: Frequency: 3 sessions/week Session duration: 45 mins (10-min warm-up, 30-min ball games, 5-min cool-down) Total intervention: 10 weeks, phased as: Adaptation → Foundation → Progression → Consolidation 3. Exercise Intensity Per ACSM's Guidelines (11th Ed.), exercise parameters for ASD individuals should progressively align with general population recommendations. Intensity calibration:Target heart rate (HR): (220 ? age) × 60%–70% = 126–147 bpm Monitoring: Polar V800 watches tracked HR in 3–5 randomly selected participants per session to ensure moderate intensity. Data included: Average HR:Minimum/mean/maximum HR (% of max) Safety protocol: Intensity was reduced if HR exceeded the safe range.
