研究目的：

本研究旨在构建基于健康行为整合理论的老年骨质疏松症伴衰弱患者运动依从性的护理干预方案，评价方案对运动依从性的影响，为医务人员开展护理干预提供参考依据。  

Objectives of Study：

This study aims to develop a nursing intervention program for improving exercise adherence in elderly patients with osteoporosis and frailty based on the Integrated Theory of Health Behavior, evaluate the impact of this program on exercise adherence, and provide a reference for medical staff to conduct nursing interventions.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）根据原发性骨质疏松症诊断与治疗指南,符合骨质疏松的诊断标准。首先使用骨密度DXA测量的中轴骨（腰椎1~4、股骨颈或全髋部）骨密度或桡骨远端1/3，经计算机自动处理后显示BMD，BMC，及BMD占同性正常对照组骨峰值的百分比及T值，最后根据临床医师开具DXA骨密度测量诊断报告获取患者骨密度DXA数据，骨密度T-值≤-2.5确诊为骨质疏松症的患者; （2）年龄在60岁以上； （3）中文版Tilburg衰弱评估量表得分≥5分； （4）神志清楚,没有明显的认知和言语交流障碍; （5）无需借助工具能够独立行走，能够自己完成适量的锻炼； （6）患者签署知情同意书,并愿意参加本次研究。

Inclusion criteria

(1) Meeting the diagnostic criteria for osteoporosis in accordance with the Guidelines for the Diagnosis and Treatment of Primary Osteoporosis [112]. First, bone mineral density (BMD) of the axial skeleton (lumbar spine 1–4, femoral neck, or total hip) or the 1/3 distal radius is measured using dual-energy X-ray absorptiometry (DXA). After automatic processing by a computer, BMD, bone mineral content (BMC), the percentage of BMD relative to the peak bone mass of the same-sex normal control group, and T-score are displayed. Finally, the patient's DXA bone density data are obtained from the DXA bone density measurement diagnostic report issued by a clinician. Patients with a bone density T-score <= -2.5 are diagnosed with osteoporosis;? (2) Aged 60 years or older [113];? (3) A score of >= 5 on the Chinese version of the Tilburg Frailty Indicator (TFI);? (4) Clear consciousness, without obvious cognitive or verbal communication impairments;? (5) Capable of independent walking without the aid of assistive devices and able to complete moderate exercise independently;? (6) Having signed the informed consent form and being willing to participate in this study.

排除标准：

（1）简易智能精神状态检查量表(Mini—Mental State Examination，MMSE)<24分； （2）最近1年内有骨折或重大手术史； （3）骨肿瘤、类风湿或因药物等导致的继发性骨质疏松； （4）无法耐受运动和禁忌体力活动的疾病，如严重心脑血管疾病、肝肾疾病、肺疾病、恶性肿瘤等； （5）正在参加其他类似研究； （6）家中已有1例患者参与本研究。

Exclusion criteria：

(1) A score of < 24 on the Mini-Mental State Examination (MMSE);? (2) A history of fracture or major surgery within the past year;? (3) Secondary osteoporosis caused by bone tumors, rheumatoid diseases, medications, or other factors;? (4) Diseases that prevent tolerance of exercise and contraindicate physical activity, such as severe cardiovascular and cerebrovascular diseases, liver and kidney diseases, pulmonary diseases, malignant tumors, etc.;? (5) Currently participating in other similar studies;? (6) Having one family member already participating in this study.
