研究目的：

本研究基于整合概念模型、症状网络视角和症状管理理论，探讨多维衰弱老年人的核心症状，了解多维衰弱老年人对核心症状的真实体验和需求，构建聚焦核心症状的多维衰弱干预方案，开展应用研究验证其效果，以期最大程度的延缓或逆转多维衰弱的发展进程，促进健康老龄化的深入实施。  

Objectives of Study：

This study, grounded in an integrative conceptual model, a symptom-network perspective, and Symptom Management Theory, investigates the core symptoms of multidimensional frailty in older adults; explores their lived experiences and needs; develops a core-symptom–focused intervention program; and conducts applied research to evaluate its effectiveness—with the aim of delaying or even reversing the progression of multidimensional frailty to the greatest extent possible and advancing healthy aging.

纳入标准：

1. 年龄≥60周岁； 2. 符合多维衰弱标准（TFI≥5分）； 3. 受试者意识清晰，能够独立或在指导下填写量表或问卷； 4. 自愿参加本研究并签署知情同意。

Inclusion criteria

1. Age >= 60 years; 2. Meets the criteria for multidimensional frailty (Tilburg Frailty Indicator, TFI >= 5); 3. Clear consciousness and able to complete scales/questionnaires independently or with guidance; 4. Willing to participate and has signed informed consent.

排除标准：

1. 躯体疾病急性期或终末期； 2. 具有严重的精神、心理、认知障碍； 3. 严重听力或语言交流障碍无法进行沟通者； 4. 运动禁忌症； 5. 正在参与其他干预项目。

Exclusion criteria：

1. In the acute or terminal phase of a physical illness; 2. Severe psychiatric/psychological or cognitive disorders; 3. Severe hearing impairment or speech/communication disorder that precludes effective communication; 4. Contraindications to exercise/physical activity; 5. Currently participating in other intervention programs.
