研究目的：

1、验证艾司氯胺酮对全喉切除术后抑郁症状的快速改善效应及维持效果； 2、整合多维度生物标志物与脑功能影像学证据，阐明艾司氯胺酮临床治疗效果的生物学关联性。  

Objectives of Study：

1. To validate the rapid improvement effect and maintenance effect of Esketamine on depressive symptoms after total laryngectomy; 2. To integrate multidimensional biomarkers with neuroimaging evidence to clarify the biological relevance of the clinical treatment effects of Esketamine.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18-75岁； 2.美国麻醉医师协会（ASA）分级I-III级； 3.喉癌或下咽癌患者且拟行全喉切除术； 4.意识良好，交流无障碍； 5.自愿参与本研究； 6.均签署知情同意书，进行宣教并配合进行数据收集。

Inclusion criteria

1. Ages 18-75; 2. American Society of Anesthesiologists (ASA) classification I-III; 3. Patients with laryngeal cancer or hypopharyngeal cancer who are scheduled for total laryngectomy; 4. Good consciousness and unobstructed communication; 5. Voluntarily participating in this study; 6. All signed informed consent forms, received education, and cooperated in data collection.

排除标准：

1.认知功能不全； 2.术前有精神疾病或使用抗精神病药物； 3.存在心肺脑功能衰竭等严重躯体疾病； 4.药物不可耐受； 5.其他任何导致不能配合研究的原因。

Exclusion criteria：

1. Cognitive dysfunction; 2. Preoperative mental illness or use of antipsychotic medication; 3. Serious physical diseases such as heart, lung, or brain failure; 4. Drug intolerance; 5. Any other reasons that prevent cooperation with the study.

麻醉诱导后30分钟内泵注艾司氯胺酮0.5 mg/kg，术后48小时内患者控制静脉镇痛(PCIA)。在PCIA中，将舒芬太尼2ug/kg联合艾氯胺酮1mg/kg用0.9%生理盐水稀释至终体积为100 ml，2ml/h自动泵注

Intervention：

Administer 0.5 mg/kg of es ketamine within 30 minutes after anesthesia induction, and provide patient-controlled intravenous analgesia (PCIA) within 48 hours post-surgery. In the PCIA, combine sufentanil 2 ug/kg with es ketamine 1 mg/kg, diluted with 0.9% saline to a final volume of 100 ml, and automatically infuse at 2 ml/h
