研究目的：

儿童青少年健康评估与促进示范项目作为辽宁省卫生援疆项目，由锦州医科大学开展实施，将在塔城地区建立“儿童青少年健康促进示范基地”。首先选择一所小学作为儿童健康促进项目示范，在以学校为中心的多层面干预背景下，以“22510SS”综合干预策略为核心，建立基于SEM的梯度防控体系。通过个体、家庭、学校、社区和卫生机构层面的针对性干预，缓解和管理儿童青少年的近视、肥胖、脊柱弯曲异常、龋齿和心理健康等问题。通过6个月干预及未来连续3年持续观察，建立儿童健康促进示范校，以期在塔城地区乃至全国推广。  

Objectives of Study：

The Child and Adolescent Health Assessment and Promotion Demonstration Project, as a health aid project of Liaoning Province to Xinjiang, is implemented by Jinzhou Medical University, and a "child and adolescent health promotion demonstration base" will be established in Tacheng area. Firstly, a primary school was selected as a demonstration of child health promotion project. Under the background of school-centered multi-level intervention, with the "22510SS" comprehensive intervention strategy as the core, a gradient prevention and control system based on SEM was established. To reduce and manage myopia, obesity, spinal deformity, dental caries and mental health problems in children and adolescents through targeted interventions at individual, family, school, community and health institution levels. Through the 6-month intervention and continuous observation for the next 3 years, a demonstration school for children's health promotion was established, with a view to promoting it in Tachheng area and even the whole country.

纳入标准：

学校标准： 1、学校校长应同意并配合项目组实施干预计划； 2、学校本身具有健康相关人力资源，如校医、健康教师、体育教师等； 3、每个年级的儿童青少年总人数应大于50人。 学生标准： 1、2-5年级，年龄6-14岁； 2、在校期间完整的体检数据； 3、一年内无转学计划。

Inclusion criteria

School Standards: 1. The principal of the school shall agree and cooperate with the project team to implement the intervention plan; 2. The school itself has health-related human resources, such as school doctors, health teachers, physical education teachers, etc. 3. The total number of children and adolescents in each grade should be more than 50. Student Standards: Grade 1, 2-5, aged 6-14 years; 2. Complete physical examination data during school; 3. No transfer plan within one year.

排除标准：

学校标准： 1、排除有才能或少数民族儿童青少年的寄宿学校和特殊学校； 2、未来1年内有明确搬迁或取消计划的学校也不包括在内。 学生标准： 1、有心脏病、高血压、糖尿病、肺结核、哮喘、肝炎、肾炎病史； 2、因疾病或药物副作用导致的肥胖； 3、身体发育异常，如侏儒症、巨人症； 4、躯体畸形，如严重的脊柱侧弯、鸡胸、跛行、明显的o型腿或x型腿； 5、近三个月内因呕吐或服药导致体重下降的； 6、眼科疾病，如角膜炎、角膜溃疡等； 7、眼科手术史，如激光手术等； 8、色弱、色盲等视觉问题。

Exclusion criteria：

School Standards: 1. Exclusion of talented or minority children and adolescents from boarding schools and special schools; 2. Schools with clear plans to relocate or cancel within the next year were also excluded. Student Standards: 1. History of heart disease, hypertension, diabetes, tuberculosis, asthma, hepatitis, nephritis; 2. Obesity due to disease or side effects of drugs; 3. Abnormal physical development, such as dwarfism and gigantism; 4. Physical deformities, such as severe scoliosis, pectus carinatum, lameness, obvious O-leg or X-leg; 5. Weight loss due to vomiting or medication within the past three months; 6. Ophthalmic diseases, such as keratitis, corneal ulcer, etc. 7. History of ophthalmic surgery, such as laser surgery; 8, weak color, color blindness and other visual problems.
