研究目的：

本研究拟通过前瞻性、随机对照试验研究，论证在SICU术后机械通气患者中使用新型苯二氮卓类药物苯磺酸瑞马唑仑在谵妄发生率及血流动力学稳定性上是否非劣于环泊酚。通过镇痛镇静工具评估导向的个体化治疗使两组达到同样的镇痛镇静目标水平，为SICU术后机械通气患者个体化镇痛镇静方案及快速康复提供更多的理论依据  

Objectives of Study：

This study aims to conduct a prospective, randomized controlled trial to demonstrate whether the novel benzodiazepine drug remimazolam besylate is non-inferior to cyclopentolate in terms of delirium incidence and hemodynamic stability in postoperative patients undergoing mechanical ventilation in the Surgical Intensive Care Unit (SICU). By using pain and sedation tools to guide personalized treatment, both groups will achieve the same levels of analgesia and sedation, providing more theoretical basis for personalized pain and sedation protocols and rapid recovery in postoperative mechanically ventilated patients in the SICU.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18岁和≤75岁； 2.病情允许维持在浅镇静状态（RASS -2~1）的术后有创机械通气患者； 3.获取患者或家属知情同意。

Inclusion criteria

1. Age between 18 and 75 years old; 2. Postoperative patients requiring invasive mechanical ventilation who can be maintained in a light sedation state (RASS -2 to 1); 3. Obtaining informed consent from the patient or their family.

排除标准：

1.神经功能障碍、精神病、痴呆或由于语言障碍无法与医护人员沟通者； 2.对研究过程用药禁忌或过敏的患者； 3.严重肝肾功能不全；

Exclusion criteria：

1. neurological dysfunction, mental illness, dementia, or individuals unable to communicate with medical staff due to language barriers; 2. patients with contraindications or allergies to study medication; 3. severe liver or kidney failure.

研究者采用随机数字表法及随机数余数分组法进行分组，即将纳入研究的重症患者进行编号，从随机数字表中任意一个数开始，沿同一方向顺序获取每个患者一个随机数字，随机数除以组数（本研究为2组）求余数并按其分组，最后根据每个组所需的实际病例数进行调整

Randomization Procedure (please state who generates the random number sequence and by what method)：

Researchers used random number table method and random number remainder grouping method for grouping. They assigned numbers to the critically ill patients included in the study, started from any number in the random number table, and obtained one random number for each patient in sequence in the same direction. They divided the patients into groups by calculating the remainder of the random number divided by the number of groups (two groups in this study) and adjusted the number of cases in each group according to the actual needs.
