研究目的：

在联合应用ACE-III量表评分与血清NfL、BDNF蛋白、IL-6、TNF-α的表达水平的方案中， 通过比较经皮穴位电刺激和星状神经节阻滞及两者联合对老年患者的神经认知功能影响作用的强弱，进而为老年患者 PND的早期预防和干预提供疗效最佳的干预措施，针对不同人群选用不同干预，以改善老年患者术后康复过程中的生活质量，从而降低其他一系列的术后并发症的发生率，促使术后快速康复理念更好地应用于临床。  

Objectives of Study：

In the protocol of combined application of ACE-III scale score and the expression levels of serum NfL, BDNF protein, IL-6, and TNF-α, by comparing the strength of the effects of transcutaneous electrical acupoint stimulation, stellate ganglion block, and their combination on the neurocognitive function of elderly patients, Furthermore, it provides the most effective intervention measures for the early prevention and intervention of PND in elderly patients. Different interventions are selected for different populations to improve the quality of life of elderly patients during the postoperative rehabilitation process, thereby reducing the incidence of a series of other postoperative complications and promoting the better application of the concept of rapid postoperative rehabilitation in clinical practice.

纳入标准：

1．在海口市人民医院择期气管插管全麻下行腹腔镜手术的患者； 2．年龄 ≥ 65 岁，性别不限； 3．ASA 分级 I～III 级； 4．BMI 18～30 ㎏/㎡； 5．本人或法定代理人签署书面知情同意书； 6．能够理解研究流程与量表内容，能够与研究人员进行有效沟通；

Inclusion criteria

1. Patients who underwent laparoscopic surgery under elective tracheal intubation and general anesthesia at Haikou People's Hospital; 2 Age >= 65 years old, gender not limited; 3. ASA classification I to III; 4. BMI: 18-30 kg /m^2; 5 I or my legal representative sign a written informed consent form; 6 Be capable of understanding the research process and scale content, and be able to communicate effectively with researchers.

排除标准：

1．严重心肺功能不全； 2．严重肝肾功能不全； 3．严重神经系统疾病史（痴呆、帕金森病、脑卒中后遗症）； 4．长期使用精神类药物或抗癫痫药物者； 5．长期服用镇静、镇痛药者； 6．有可能因为TEAS而出现医疗问题的患者，例如，所选穴位局部有皮肤破损者；有心脏起搏器或金属植入物或对穴位刺激电极贴过敏的患者； 7．对盐酸罗哌卡因过敏者； 8．存在凝血功能障碍者(PT（凝血酶原时间）>ULN+3s 和/或 APTT（活化部分凝血活酶 时间）>ULN+10s); 9．视觉与听觉障碍无法交流者； 10．主观或客观原因不愿意配合者； 11．术前ACE-III评分，评判存在认知功能障碍者（评分小于88分）；

Exclusion criteria：

1. Severe cardiopulmonary insufficiency; 2 Severe liver and kidney dysfunction; 3 History of severe neurological diseases (dementia, Parkinson's disease, sequelae of stroke); 4 Those who have been using psychotropic drugs or antiepileptic drugs for a long time; 5 Those who have been taking sedatives and analgesics for a long time; 6 Patients who may encounter medical problems due to TEAS, for example, those with local skin damage at the selected acupoints; Patients with pacemakers or metal implants, or those allergic to acupoint stimulation electrode patches; 7 People allergic to ropivacaine hydrochloride; 8 Those with coagulation disorders (PT (prothrombin time) >ULN+3 seconds and/or APTT (activated partial thromboplastin) Time >ULN+10 seconds; 9 People with visual and auditory impairments who are unable to communicate; 10 Those who are unwilling to cooperate for subjective or objective reasons; 11 Preoperative ACE-III score was used to evaluate those with cognitive dysfunction (score less than 88 points);

在麻醉诱导前 30min，选取内关穴（PC6，位于桡侧腕屈肌腱和掌长肌肌腱两个肌腱之间，前臂掌侧，横腕纹2寸处）、合谷穴（LI4，位于手背部第二掌骨桡侧中点处，手背第一、二掌骨之间）、足三里穴（ST36，位于小腿的前外侧方，犊鼻穴（外膝眼）下4横指，离胫骨前嵴1横指处），用75%酒精擦拭脱脂（减少皮肤阻抗）并擦干，将凝胶电极贴紧压贴于PC6、L14、STC6穴位上， 并连接电刺激仪，打开刺激仪开关后调节刺激参数：刺激时间为从麻醉诱导前30min开始至手术结束后停止；模式选择疏密波，刺激频率选择 2/100Hz，刺激强度选择患者所能耐受（感觉到有明显的震颇感但无疼痛感）的最大刺激强度为宜。

Intervention：

Thirty minutes before anesthesia induction, the Neiguan point (PC6, located between the flexor carpi radialis tendon and the longus palmaris tendon, on the palmar side of the forearm, 2 cun from the transverse wrist crease), the Hegu point (LI4, located at the midpoint of the radial side of the second metacarpal bone on the back of the hand, between the first and second metacarpal bones on the back of the hand), and the Zusanli point (ST36, located on the anterolateral side of the lower leg 4 transverse fingers below the Dubi point (outer knee eye), 1 transverse finger away from the anterior tibial ridge, wipe with 75% alcohol to remove fat (reduce skin resistance) and dry. Firmly press the gel electrode onto the PC6, L14, and STC6 acupoints, and connect the electrical stimulator. After turning on the stimulator switch, adjust the stimulation parameters: The stimulation period starts 30 minutes before anesthesia induction and ends after the operation. The mode is selected as sparse and dense waves, the stimulation frequency is chosen as 2/100Hz, and the stimulation intensity is selected as the maximum stimulation intensity that the patient can tolerate (feeling a distinct vibration sensation but no pain)

在麻醉诱导前30min，选取与T组相同的穴位贴凝胶电极片但不予刺激。在麻醉诱导前10min，进行超声引导下右侧星状神经节阻滞，注入0.2% 盐酸罗哌卡因注射液5 ml，以出现右侧 Horner’s 综合征（瞳孔缩小、上睑下垂、眼球内陷、同侧面部无汗以及结膜充血) 为阻滞成功的标志；

Ten minutes before anesthesia induction, ultrasound-guided right stular ganglion block was performed, with 5 ml of 0.2% ropivacaine hydrochloride injection injected. The appearance of right Horner's syndrome (pupil constriction, ptosis, enophthalmus, anhidrosis on the same side, and conjunctival congestion) was regarded as a sign of successful block.
