Objectives of Study：

To evaluate the feasibility and acceptability of the intervention program, as well as the usabilityof the self-management support system

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.>=60岁的老年人； 2.无重大疾病能够长期接受随访、配合完成调查； 3.有移动智能手机、移动平板等电子移动设备；④能独立签署知情同意书。

Inclusion criteria

1. >=60 years old; 2. No major diseases can be followed up for a long time and cooperate with the completion of the investigation; 3. There are electronic mobile devices such as mobile smartphones and mobile tablets; (4) Be able to independently sign the informed consent form.

排除标准：

1.存在精神疾病、严重认知障碍者； 2.严重躯体障碍需长期卧床者； 3.目前正在参加其他临床试验者。

Exclusion criteria：

1. Those with mental illness or severe cognitive impairment; 2. Those with severe physical disabilities who need to be bedridden for a long time; 3. Those who are currently participating in other clinical trials.
