研究目的：

比较瑞马唑仑相较于丙泊酚在老年患者胃肠EMR术中，是否有更好的血流动力稳定性和术后恢复质量。  

Objectives of Study：

To determine whether remimazolam provides superior hemodynamic stability and higher postoperative recovery quality compared to propofol in elderly patients undergoing gastrointestinal endoscopic mucosal resection (EMR).

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄>=65岁且<=80岁，拟行胃肠EMR术（波士顿评分>=6分）; 2.ASA分级Ⅱ-Ⅲ级，基线MMSE>=24分（文盲>=17分）; 3.体重指数（BMI）18～28 kg/m^2; 4.自愿签署知情同意书。

Inclusion criteria

1. Age between 65 and 80 years, planned for gastrointestinal EMR (Boston score >= 6 points); 2. ASA classification II-III, baseline MMSE >= 24 points (illiterate >= 17 points); 3. Body mass index (BMI) 18-28 kg/m^2; 4. Voluntarily signed informed consent.

排除标准：

1.对苯二氮?类或丙泊酚过敏； 2.严重肝肾功能不全（Child-Pugh C级或eGFR<30 mL/min/1.73m^2）； 3.严重心血管疾病（NYHA心功能Ⅲ-Ⅳ级，不稳定性心绞痛）； 4.困难气道或高通气风险； 5.长期使用镇静、阿片类药物； 6.认知功能障碍（MMSE<24分）； 7.急诊手术。

Exclusion criteria：

1. Allergy to benzodiazepines or propofol; 2. Severe liver and kidney dysfunction (Child-Pugh Class C or eGFR <30 mL/min/1.73m^2); 3. Severe cardiovascular disease (NYHA Class III-IV heart failure, unstable angina); 4. Difficult airway or high ventilation risk; 5. Long-term use of sedatives or opioids; 6. Cognitive dysfunction (MMSE <24 points); 7. Emergency surgery.
