研究目的：

比较氢吗啡酮缓释片联合地舒单抗与羟考酮缓释片联合地舒单抗治疗实体瘤骨转移的疗效和安全性。探索与疗效和安全性相关的潜在生物标志物，检测观察骨转移癌生物标志物的变化情况，为临床选择治疗癌痛的最佳途径提供依据和参考。  

Objectives of Study：

Compare the efficacy and safety of hydromorphone sustained-release tablets combined with denosumab and hydrocodone sustained-release tablets combined with denosumab in the treatment of solid tumor bone metastasis. Explore potential biomarkers related to efficacy and safety, detect and observe changes in biomarkers of bone metastatic cancer, and provide a basis and reference for clinical selection of the best approach to treat cancer pain.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 经组织学或细胞学确诊的实体瘤患者，且存在影像学证实的骨转移； 2. 年龄在 18 - 75 岁之间； 3. 疼痛数字评分法（NRS）评分>= 4 分，且疼痛主要由骨转移引起； 4. 骨转移数量>=3处； 5. 预计生存期>=3 个月。 6. 肝肾功能基本正常（血清肌酐清除率>= 30ml/min，总胆红素<=1.5 倍正常值上限，谷丙转氨酶和谷草转氨酶<=3 倍正常值上限）； 7. 可经口服给药； 8. 患者签署同意书。

Inclusion criteria

1. Patients with solid tumors diagnosed through histology or cytology, and confirmed bone metastases by imaging; 2. Age between 18 and 75 years; 3. Pain Numeric Rating Scale (NRS) score >= 4, with pain primarily caused by bone metastases; 4. At least 3 sites of bone metastases; 5. Expected survival period >= 3 months. 6. Liver and kidney function basically normal (serum creatinine clearance rate >= 30ml/min, total bilirubin <= 1.5 times the upper limit of normal, alanine aminotransferase and aspartate aminotransferase <= 3 times the upper limit of normal); 7. Able to take medication orally; 8. Patient has signed a consent form.

排除标准：

1. 对氢吗啡酮、羟考酮缓释片或地舒单抗过敏者; 2. 正在接受其他试验性药物治疗或在规定洗脱期内使用过其他强效阿片类药物（如洗脱期不足7天）; 3. 研究前7天接受过骨水泥治疗、放化疗、同位素治疗等与疼痛相关的治疗的患者; 4. 存在严重的精神疾病或认知障碍，无法配合研究评估; 5. 有严重的心肺功能不全（纽约心脏病协会心功能分级 IV 级或无法纠正的呼吸衰竭）; 6. 存在其他严重的合并症或感染，可能影响研究结果或增加治疗风险; 7. 研究者认为其他不适合纳入的情况。

Exclusion criteria：

1. Those allergic to hydromorphone, oxycodone extended-release tablets, or datopotamab; 2. Currently receiving treatment with other experimental drugs or have used other potent opioids during the designated washout period (if the washout period is less than 7 days); 3. Patients who have received cementation treatment, radiotherapy, chemotherapy, or other pain-related treatments within 7 days prior to the study; 4. Individuals with severe mental illness or cognitive impairment that prevents cooperation in the study evaluation; 5. Patients with severe cardiopulmonary dysfunction (New York Heart Association functional classification IV or uncorrectable respiratory failure); 6. Presence of other serious comorbidities or infections that may affect study results or increase treatment risks; 7. Other conditions deemed inappropriate for inclusion by the investigator.
