研究目的：

研究目的：探讨益生菌不同给药途径对回肠造口术后患者肠功能恢复情况，比较两种方法的安全性与有效性。 (1)研究和探讨经口服或回肠造口不同给药途径行益生菌灌肠治疗的疗效，加快术后肠功能恢复，降低术后肠功能障碍等并发症的发生，减少总住院时间，加速患者康复。 (2).研究采用经回肠造口行顺行益生菌灌肠治疗的安全性不劣于口服给药组。 (3).研究明确回肠造口行顺行益生菌灌肠治疗后促进术后肠功能恢复的有效性。 (4)发表核心期刊论文一篇。  

Objectives of Study：

To explore the recovery of intestinal function in patients after ileostomy due to different administration routes of probiotics, and to compare the safety and efficacy of the two methods.

纳入标准：

①诊断回肠造口并符合回纳手术指征者并同意行手术治疗，其性别、种族、术中麻醉方式不限；② 80岁>年龄≥ 18 岁且患者自愿接受手术.③术后生命体征稳定，无严重感染或肠梗阻。④术前肠道准备完整、无严重合并症者;

Inclusion criteria

1 For those diagnosed with ileostomy and meeting the indications for repositioning surgery, and who agree to undergo surgical treatment, there are no restrictions on their gender, race, or intraoperative anesthesia method. 2 Patients aged 18 to 80 years old who are willing to undergo surgery. 3 Postoperative vital signs are stable, and there is no severe infection or intestinal obstruction. 4 Those with complete preoperative intestinal preparation and no serious complications;

排除标准：

①未能配合治疗方案用药、复诊者；②严重系统性疾病者；③对益生菌过敏、合并免疫缺陷或肠道恶性肿瘤复发者。④既往严重胃肠道疾病⑤术后必须使用对胃肠功能有明显影响的药物或治疗者⑥术中并发严重出血、脏器损伤者。

Exclusion criteria：

1 Those who fail to cooperate with the treatment plan for medication and follow-up visits; 2 People with severe systemic diseases; 3 Those who are allergic to probiotics, have concurrent immune deficiencies or have relapsed intestinal malignant tumors. 4 Those with severe gastrointestinal diseases in the past 5 Those who must use drugs or treatments that have a significant impact on gastrointestinal function after the operation 6 Those who have severe bleeding or organ damage during the operation.

术前接受顺行益生菌灌肠（如双歧杆菌三联活菌肠溶胶囊210mg*8粒；每克含长型双歧杆菌≥1.0*10^6 CFU，嗜酸乳杆菌≥1.0*10^6 CFU，粪肠球菌≥1.0*10^6 CFU，每日1次）（每日1次，持续3天）；协助患者平卧位，暴露回肠造口，指导患者及家属判断造口近远端开口，戴上手套，蘸取石蜡油后探明远端造口开口，明确造口走向，将益生菌和生理盐水混合大约200ml，混合后的混悬液用50ml针筒抽取混悬液后连接导尿管，将充好气囊的导尿管放入远端造口内，从回肠造口插入至合适位置（10-20 cm），将混悬液经尿管灌入远端回肠内，缓慢灌入益生菌液（速度控制在100 ml/10 min左右），以无逆流为宜，每次灌入200 ml益生菌溶液，灌肠后留置10分钟后拔除，观察患者情况。

Before the operation, receive anterograde probiotic enema (such as Bifidobacterium triplex Live Bacteria enteric-coated Capsules 210mg*8 capsules; each gram contains Bifidobacterium longum >= 1.0*10^6 CFU, Lactobacillus acidophilus >= 1.0*10^6 CFU, Enterococcus faecalis >= 1.0*10^6 CFU, once a day) (once a day for 3 consecutive days). Assist the patient in a lying position, expose the ileostomy, guide the patient and their family to determine the proximal and distal openings of the stoma, put on gloves, dip paraffin oil to identify the distal opening of the stoma, and clarify the direction of the stoma. Mix approximately 200ml of probiotics and normal saline. Use a 50ml syringe to draw the suspension and connect it to the catheter. Place the catheter filled with a balloon into the distal stoma and insert it into the appropriate position (10-20 cm) through the ileostoma. Pour the suspension into the distal ileum through the catheter, and slowly inject the probiotic solution (at a speed of about 100 ml per 10 minutes), ensuring there is no reflux. Each time, inject 200 ml of the probiotic solution. After enema, the enema was left in place for 10 minutes and then removed to observe the patient's condition.
