研究目的：

通过将磁共振 T1 定量图谱和 DCEP 的整套处理方案融合，结合其他 MRI 定量图谱，前瞻性、双中心建立人体 MRI 整套扫描协议，评价慢性肝病中主要病种脂肪变性肝病患者肝脏“自由呼吸状态下使用 3D Radial Stack-of-stars 快速扰相梯度回波序列进行 T1 定量图谱和 DCEP”的有效性，并与一般临床资料、实验室检查及部分患者穿刺活检病理结果结合、对照，进一步探索和再评价该技术在配合不佳患者中的优势，提供循证依据。此外，拓展该方法在肾脏和胰腺的定量评估。  

Objectives of Study：

By integrating the quantitative T1 mapping of magnetic resonance imaging with the complete processing protocol of DCEP, and combining it with other MRI quantitative mappings, a prospective, dual-center study was conducted to establish a comprehensive MRI scanning protocol for the human body. This protocol aims to evaluate the effectiveness of using the 3D Radial Stack-of-stars fast spoiled gradient echo sequence for T1 quantitative mapping and DCEP in patients with hepatic steatosis, a major condition in chronic liver disease, under free-breathing conditions. The study correlates and contrasts these findings with general clinical data, laboratory tests, and pathological results from biopsy samples of some patients, further exploring and reassessing the advantages of this technique in patients with poor compliance, thereby providing evidence-based support. Additionally, the study extends the application of this method to the quantitative assessment of the kidneys and pancreas.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

脂肪肝病受试者纳入标准：于上海市中医医院和上海中医药大学附属曙光医院脂肪肝专病门诊就诊，根据 2023 年国际临床诊断标准[A multi-society Delphi consensus statement on new fatty liver disease nomenclature. Hepatology.2023;78(6):1966-1986]，通过影像学或先前病理诊断为 CLD（SLD） 的患者，同意接受血液检测和 MRI 检查。如愿意接受肝活检患者，在活检前 3 天内进行 MRI 扫描和血液检测。 健康志愿者纳入标准：符合纳入基本准则，无慢性肝病(脂肪肝、肝炎、肝硬化)病史及临床症状；肝功能正常；腹部MRI-PDFF显示肝脏脂肪含量＜5%；无长期饮酒病史；近6月内未使用影响肝脏代谢的药物；自愿签署知情同意书

Inclusion criteria

Inclusion criteria for subjects with fatty liver disease: Attended the fatty liver disease clinic of Shanghai Hospital of Traditional Chinese Medicine and Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine, according to the 2023 international clinical diagnostic criteria [A multi-society Delphi consensus statement on new fatty liver disease nomenclature. Hepatology.2023; 78(6):1966-1986], diagnosed with CLD (SLD) by imaging or prior pathology , agreeing to undergo blood tests and MRI examinations. If willing to undergo liver biopsy, MRI scan and blood tests are performed within 3 days prior to biopsy. Inclusion criteria for healthy volunteers: meet the basic inclusion criteria, no history and clinical symptoms of chronic liver disease (fatty liver, hepatitis, cirrhosis); Normal liver function; Abdominal MRI-PDFF showed liver fat content <5%; No history of long-term alcohol consumption; No use of drugs affecting liver metabolism in the past 6 months; Voluntarily sign the informed consent form

排除标准：

无法配合或耐受该检查者；有MRI检查禁忌证及影响图像质量的因素受试者；有精神疾病、肿瘤疾病或严重心血管疾病的受试者；妊娠妇女；没有血液检查结果；肝脏内较大的占位性病变（长轴直径>20mm）；恶性肿瘤；肾功能不全，肾小球滤过率小于 60 ml/min×1.73m^2；心功能不全。

Exclusion criteria：

Those who cannot cooperate with or tolerate the examination; Subjects with contraindications to MRI examination and factors affecting image quality; Subjects with mental illness, oncological disease, or severe cardiovascular disease; Pregnant women; No blood test results; Large space-occupying lesions in the liver (long axis diameter > 20mm); Malignancy; renal insufficiency, glomerular filtration rate less than 60 ml/min×1.73m^2; cardiac insufficiency.
