研究目的：

本研究旨在探索家庭-社区-二级医院-三级医院的多层级照护模式对于居家腹膜透析患者自我管理能力、生活质量以及透析效果的影响。  

Objectives of Study：

This study aimed to explore the impact of the "three elements" care model of home-secondary hospital-tertiary hospital on the self-management ability, quality of life and dialysis outcomes of home-based peritoneal dialysis patients.

纳入标准：

1、患者必须接受过三个月以上的维持性腹膜透析，其中75%以上的治疗在家中完成； 2、患者必须拥有智能手机，并能熟练使用； 3、自愿参与本研究； 4、参与者必须年满18周岁。

Inclusion criteria

1, patients must have received more than three months of maintenance peritoneal dialysis, with more than 75% of the treatments completed at home; 2, patients must have a smartphone and be proficient in its use; 3, voluntary participation in this study; 4, participants must be at least 18 years old.

排除标准：

1、同时接受血液透析的患者； 2、有认知障碍或精神障碍的患者； 3、经评估患有严重疾病或无法参与研究的患者。

Exclusion criteria：

1, those who are also receiving haemodialysis; 2, those with cognitive and psychiatric disorders; 3, those who are assessed to be severely ill or unable to cooperate with this study

对照组患者接受常规的居家照护模式，主要包括以下内容：i. 医院随访：患者定期到二级或三级医院进行随访，由肾内科的医生和腹膜透析专科护士对患者进行评估和指导；ii. 患者教育：由腹膜透析专科护士向患者提供腹膜透析操作指导以及相关注意事项的健康教育；iii. 电话随访：医院护士每月通过电话随访了解患者的透析情况以及健康状况，并及时与医生沟通动态调整患者的治疗方案。

Patients in the control group received a conventional home care model comprising the following components: i. Hospital Follow-up: Patients attended regular follow-ups at secondary or tertiary hospitals, where they were assessed by nephrologists and peritoneal dialysis (PD) nurses. ii. Patient Education: PD nurse specialists provided patients with guidance on the procedures of PD and educated them about relevant safety precautions. iii. Telephone Follow-up: Hospital nurses conducted monthly follow-ups via telephone to monitor patients' dialysis and health conditions, communicating promptly with doctors to facilitate timely adjustments to patients' treatment plans.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.数据采集方法 本研究采用多种数据采集方式：包括患者及其家庭成员通过问卷调查（如腹膜透析患者自我管理能力量表和肾病生活质量简表KDQOL-36?）、访视收集及医疗记录抽取生化指标（血红蛋白、血钙、血磷）等方式。数据由两名护理硕士研究生和一名临床护士在试验前及6个月后随访时采集。 2.数据管理流程 所有数据录入EpiData 3.1数据库，采取双独立数据录入及对比，确保数据录入准确性。使用SPSS 26.0进行统计分析。数据经过逻辑校验和连续质量控制，确保数据完整可靠。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Data collection methods A variety of data collection methods were used in this study: including patients and their family members through questionnaires (e.g., Peritoneal Dialysis Patient Self-Management Competency Scale and Kidney Disease Quality of Life Short Form, KDQOL-36?), visit collection, and biochemicals (haemoglobin, calcium and phosphorus) extracted from medical records.Data were collected by two Masters of Nursing students and one clinical nurse before the trial and at the follow-up visit after 6 months. 2.Data management process All data were entered into the EpiData 3.1 database, and dual independent data entry and comparison was adopted to ensure data entry accuracy.Statistical analyses were performed using SPSS 26.0.Data were logically calibrated and continuously quality controlled to ensure data integrity and reliability.
