研究目的：

1.开展真实世界临床研究，探索基于数字疗法的生活方式干预能否提高CMD患者行为依从性及降低主要不良心脑血管事件（MACCE）的发生率及再住院率。 2.评估基于数字疗法的心血管代谢性疾病人群生活方式干预模式的实施情况，包括有效性、规范性、可接受性、采用性、恰当性、可行性、实施成本、覆盖度（普及率）和可持续性，以期为防治心血管代谢性疾病提供新的临床证据和策略支持。  

Objectives of Study：

1. Conduct real-world clinical studies to explore whether digital therapy-based lifestyle intervention can improve behavioral adherence and reduce the incidence and rehospitalization rate of major adverse cardiovascular and cerebrovascular events (MACCE) in CMD patients. 2. Evaluate the implementation of the digital therapeutics-based lifestyle intervention model for people with cardiometabolic diseases, including effectiveness, normativeness, acceptability, adoption, appropriateness, feasibility, implementation cost, coverage (penetration), and sustainability, in order to provide new clinical evidence and strategic support for the prevention and treatment of cardiometabolic diseases.

纳入标准：

1.年龄18-80岁； 2.诊断为心血管代谢性疾病（包括高血压、高脂血症、冠心病、肥胖、代谢综合征、睡眠呼吸暂停等）； 3.会使用智能手机及微信小程序； 4.签署知情同意书。

Inclusion criteria

1. Age 18-80 years old; 2. Diagnosed with cardiometabolic diseases (including hypertension, hyperlipidemia, coronary heart disease, obesity, metabolic syndrome, sleep apnea, etc.); 3. Be able to use smartphones and WeChat mini programs; 4. Sign the informed consent form.

排除标准：

1.急性感染性疾病、炎症性疾病（如炎症性肠病，血管炎、类风湿性关节炎、系统性红斑狼疮等活动期风湿免疫性疾病）； 2.沟通障碍或严重的精神心理疾病； 3.接受肾脏替代治疗的慢性肾脏病患者； 4.孕妇/哺乳期妇女； 5.无智能手机或无法正常使用智能手机及微信小程序； 6.生活无法自理，不能签署知情同意书的患者； 7.不愿意配合者。

Exclusion criteria：

1. Acute infectious diseases and inflammatory diseases (such as inflammatory bowel disease, vasculitis, rheumatoid arthritis, systemic lupus erythematosus and other active rheumatic immune diseases); 2. Communication disorders or severe mental and psychological diseases; 3. Patients with chronic kidney disease receiving renal replacement therapy; 4. Pregnant/lactating women; 5. No smartphone or unable to use smartphone and WeChat mini program normally; 6. Patients who are unable to take care of themselves and cannot sign the informed consent form; 7. Those who are unwilling to cooperate.
