研究目的:

通过建立和评估一种改良的局部效应模型,探讨剂量加权平均传能线密度(LETd)引导下碳离子放疗应用于局部晚期胰腺癌的临床安全性及有效性。  

Objectives of Study:

This study investigates the clinical safety and efficacy of LETd-guided carbon ion radiotherapy for locally advanced pancreatic cancer by developing and evaluating a modified local effect model.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.有能力依从方案并签署知情同意书;
2.年龄≥18岁;
3.组织或细胞病理学确诊的胰腺导管腺癌;
4.根据NCCN胰腺癌指南2025第2版定义为局部晚期不可手术切除者;
5.经影像学检查评估排除远处转移,但腹主动脉旁淋巴结转移可包括在放射野内者可入组;
6.胰腺原发病灶或阳性淋巴结未侵犯消化道;
7.ECOG一般状态评分为0-1分;
8.足够的骨髓及肝肾功能;

Inclusion criteria

1. Ability to comply with the study protocol and provide written informed consent; 2. Aged between 18 and 80 years; 3. Histologically or cytologically confirmed pancreatic ductal adenocarcinoma (PDAC); 4. Defined as locally advanced unresectable diseases according to the NCCN Guidelines Pancreatic Adenocarcinoma Guidelines (Version 2.2025); 5. No evidence of distant metastasis based on imaging examinations; 6. The primary lesion or positive lymph nodes do not invade the gastrointestinal tract; 7. ECOG performance status score of 0–1; 8. Adequate bone marrow, as well as hepatic and renal function;

排除标准:

1.肿瘤已侵犯邻近消化道(特别是已侵犯粘膜表面);
2.过去曾经接受过上腹部放疗,或放射性粒子植入,可能导致照射野重叠;
3.过去曾经接受过其他的胰腺癌局部治疗方法,如高强度聚焦超声(HIFU)、不可逆电穿孔(纳米刀)等;
4.存在可能受高能射线影响干扰其正常功能或可能影响放疗靶区剂量的体内植入物;
5.无法在体位固定模具上俯卧15分钟以上;
6.先前未行化疗或化疗方案不包含吉西他滨或氟尿嘧啶类药物;
7.存在活动性胃和/或十二指肠溃疡、上腹部活动性感染、活动性炎症性肠病,或其他严重合并症;
8.伴其他恶性肿瘤病史;

Exclusion criteria:

1. Tumor invaded adjacent gastrointestinal structures; 2. History of prior radiotherapy to the upper abdomen or radioactive seed implantation that may lead to overlapping radiation fields; 3. Previous local therapies for pancreatic cancer such as high-intensity focused ultrasound or irreversible electroporation, etc. 4. Presence of internal implanted that may malfunction under high-energy radiation exposure or interfere with radiation dose distribution; 5. Unable to lie prone for more than 15 minutes on the immobilization device; 6. Prior chemotherapy regimen not including gemcitabine or fluoropyrimidines; 7. Presence of active gastric and/or duodenal ulcers, active upper abdominal infections, active inflammatory bowel disease, or other severe comorbidities; 8. History of other malignancies;
