研究目的：

基于JBI循证卫生保健模式理论及出院准备概念模型，通过文献回顾形成老年前列腺癌根治术患者出院准备服务最佳证据总结，根据证据总结内容，从全程视角和整体性视角构建老年前列腺癌根治术后患者出院准备度干预方案，并验证干预方案的应用效果，以期提高老年前列腺癌根治术后患者的出院准备度、出院指导质量及生活质量。  

Objectives of Study：

Based on the theory of JBI evidence-based health care model and the conceptual model of discharge readiness, a summary of the best evidence on discharge readiness ser-vices for elderly patients undergoing radical prostatectomy was formed through a litera-ture review. Based on the content of the summary of the evidence, an intervention pro-gram on discharge readiness for elderly patients undergoing radical prostatectomy was constructed from the whole process perspective and holistic perspective, and the effec-tiveness of the program was verified, with the aim of improving the discharge readiness, quality of discharge instructions and quality of life of elderly patients after radical pros-tatectomy.

纳入标准：

1.年龄≥60岁； 2.经前列腺穿刺活检，病理确诊为前列腺癌后行前列腺癌根治术； 3.具备沟通能力； 4.知情同意且自愿参加本研究。

Inclusion criteria

1. Age >= 60 years; 2. Patients who underwent radical prostatectomy after prostate biopsy and were pathologically diagnosed with prostate cancer; 3. Have communication skills; 4. Have given informed consent and are willing to participate in this study.

排除标准：

1.具有认知功能障碍、语言表达障碍、或视听觉障碍； 2.病情严重以致不能参加； 3.患者本人要求退出研究； 4.研究过程中因更换联系方式或死亡而无法联系。

Exclusion criteria：

1. Having cognitive dysfunction, language expression disorder, or visual and auditory impairment; 2. The condition is so severe that they cannot participate; 3. The patient requests to withdraw from the study; 4. Unable to be contacted during the study due to changes in contact information or death.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

筛选符合标准的患者后，向患者介绍本研究的目的、意义，请患者签署“知情同意书”。在研究者的指导下，请患者根据实际情况填写调查表或者研究者以中性语气向患者阅读问题及可供选择的选项，患者作答，研究者帮其填写。所有的研究成员都将对患者的身份保密。档案将保存在泌尿外科病区档案柜中，仅供研究人员查阅。研究组成员在法律允许的范围内，尽一切努力保护患者的医疗资料的隐私。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

After screening the patients who meet the criteria, introduce the purpose and significance of this study to the patients, and ask them to sign the "Informed Consent Form". Under the guidance of the researchers, ask the patients to fill out the questionnaire according to the actual situation or let the researchers read the questions and the available options to the patients in a neutral tone, and have the patients answer. The researchers will help them fill it out. All the research members will keep the patients' identities confidential. The files will be stored in the archive cabinet of the urology ward and can only be accessed by the researchers. The members of the research group will do their best to protect the privacy of the patients' medical information within the legal limits.
