研究目的：

本研究将经鼻高流量吸氧（HFNC）应用于高碳酸血症COPD患者的无痛支气管镜检查中，探索此类患者在行无痛支气管镜治疗时最适宜的氧流量，为临床应用提供参考。  

Objectives of Study：

In this study, high-flow nasal cannula (HFNC) was applied to painless bronchoscopy in patients with hypercapnic chronic obstructive pulmonary disease (COPD), to explore the most appropriate oxygen flow rate during painless bronchoscopy in such patients, and to provide reference for clinical application.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合慢性阻塞性肺疾病COPD的诊断标准[22]；经检查后发现二氧化碳分压 (PaCO2 )>50 mmHg,确诊为合并高碳酸血症者; （2）美国麻醉医师协会（American Society of Anesthesiologists，ASA）分级Ⅰ~Ⅲ级。

Inclusion criteria

1.Meet the diagnostic criteria of chronic obstructive pulmonary disease COPD; The partial pressure of carbon dioxide was found on examination(PaCO2)>50 mmHg, diagnosed with hypercapnia; 2.American Society of Anesthesiologists (ASA) grade I-III.

排除标准：

（1）不配合或不耐受HFNC的患者； （2）颅底骨折或鼻骨骨折； （3）年龄≥90岁或≤18岁； （4）上呼吸道完全阻塞； （5）经由麻醉医生和呼吸科医生评估不适合纳入研究病情危重的患者。

Exclusion criteria：

1.Patients who do not cooperate with or tolerate HFNC; 2.Skull base fracture or nasal bone fracture; 3.Age >=90 or <=18 years old; 4.Complete obstruction of the upper airway; 5.Critically ill patients who were deemed ineligible for study by anesthesiologists or respiratory physicians.

盲法：

研究过程采用双盲法，由独立麻醉医师开封并设置氧流量，操作者、患者及观察者均不知分组信息；HFNC流量显示屏遮挡，仅显示“模式A/B/C”。由同一名呼吸内科医师进行气管镜诊疗相关操作。另一名有经验的麻醉医师作为盲态观察者负责麻醉维持、监护术中生命体征，保证合适的镇静深度并记录患者基本信息、气管镜检查具体操作，以及检查过程中的各项观察指标、不良反应发生情况。

Blinding：

The study was performed in a double-blind manner, and the oxygen flow rate was set by an independent anesthesiologist. The operator, patients and observers were unaware of the group information. The HFNC flow display is occluded and only "Mode A/B/C" is displayed. Bronchoscopy was performed by the same respiratory physician. Another experienced anesthesiologist, as a blind observer, was responsible for the maintenance of anesthesia, monitoring intraoperative vital signs, ensuring the appropriate depth of sedation, and recording the basic information of patients, the specific operation of bronchoscopy, and the observation indicators and adverse reactions during the examination.
