研究目的：

以常规静推肝素的方式为对照，探讨基于高压滴注持续导管内动脉应用肝素在急性大血管闭塞脑卒中机械取栓术中的有效性和安全性，为临床提供一种机械取栓术中肝素应用的新选择  

Objectives of Study：

To investigate the efficacy and safety of continuous intra-catheter arterial application of heparin based on high pressure infusion in mechanical thrombectomy for stroke patients with acute large vessel occlusion, and to provide a new option for clinical application of heparin in mechanical thrombectomy

纳入标准：

根据以下标准纳入因大脑大动脉近端闭塞导致的 AIS 拟行机械取栓术的患者： 1.年龄≥18 岁，初始美国国立卫生研究院卒中量表（NIHSS）评分＞5； 2.指数卒中前改良Rankin量表(mRS)≤2； 3.当临床症状严重经影像学检查明确脑近端动脉闭塞，包括颅内颈内动脉(ICA)、大脑中动脉 (MCA) 的 M1/M2 段、基底动脉 (BA) 闭塞或优势椎动脉 (VA) 闭塞（功能性椎动脉闭塞），串联颅外 ICA 和颅内大动脉闭塞； 4.符合条件的急性大血管闭塞行MT患者，超时间窗（6-24h）患者符合Dawn和defuse3标准； 5.对于中风发作后4.5小时内的患者，根据现行指南，在准备 MT（直接桥接IVT治疗）时进行静脉溶栓 (IVT)。如果禁忌或拒绝IVT，则直接进行MT； 6.征得受试者或其法定授权代表的知情同意； 7.前循环卒中 (ACS) 的起始穿刺 (OTP) 时间 <12 小时，后循环卒中 (PCS) <24 小时；

Inclusion criteria

Patients with AIS due to proximal cerebral artery occlusion were enrolled for mechanical thrombectomy according to the following criteria: 1. Age >=18 years, initial National Institutes of Health Stroke Scale (NIHSS) score >=5; 2. index pre-stroke modified Rankin scale (mRS)<=2; 3 When the clinical symptoms were severe, the proximal cerebral artery occlusion was confirmed by imaging examination, including intracranial internal carotid artery (ICA), M1/M2 segment of middle cerebral artery (MCA), basilar artery (BA) occlusion or dominant vertebral artery (VA) occlusion (functional vertebral artery occlusion), and tandem extracranial ICA and intracranial large artery occlusion were also included. 4. Eligible patients with acute large vessel occlusion undergoing MT, patients beyond the time window (6-24h) met the Dawn and defuse3 criteria. 5. For patients within 4.5 hours after stroke onset, intravenous thrombolysis (IVT) was performed when MT (direct bridging IVT therapy) was prepared according to current guidelines. If IVT was contraindicated or refused, MT was performed directly. 6. Informed consent was obtained from the subjects or their legally authorized representatives. 7.The onset-to-puncture (OTP) time for anterior circulation stroke (ACS) is less than 12 hours, while for posterior circulation stroke (PCS), it is less than 24 hours.

排除标准：

1.NIHSS评分≤5分；
2.CT或MRI 扫描中线移位显著的占位效应；
3.不可能进行90天的随访；
4.单纯动脉溶栓以及其他任何原因不能进行MT的患者；
5.存在应用肝素禁忌症；
6.研究者认为不适宜参加本临床研究；

Exclusion criteria：

1. NIHSS score <=5; 2. mass effect with significant midline shift on CT or MRI scan; 3. It was not possible to follow up for 90 days; 4. patients with arterial thrombolysis alone and those who could not undergo MT for any other reason; 5. Patients with contraindications to heparin use; 6. The investigators considered that they were not suitable for participating in this clinical study.
