研究目的：

1.探究分级运动康复、氧化应激水平和膈肌功能变化三者之间的内在相关性。 2.分析氧化应激水平、膈肌功能变化与AECOPD患者肺功能、运动耐力与生活质量等临床症状之间的关联性。  

Objectives of Study：

1. Investigate the intrinsic correlation between graded exercise rehabilitation, oxidative stress levels, and diaphragmatic function changes. 2. Analyze the correlation between oxidative stress levels, diaphragmatic function changes, and clinical symptoms such as lung function, exercise endurance, and quality of life in patients with AECOPD.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、根据GOLD（Global Initiative for Chronic Obstructive Lung Disease，慢阻肺全球倡议）2024版中COPD的诊断标准，且病情处于急性加重期； 2、满足2022年EXODUS（EXpert consensus On Diaphragm Ultra Sonography in the critically ill，重症膈肌超声检测专家共识）膈肌移动度＜2cm，膈肌厚度＜2.2mm，膈肌增厚分数＜20%中任意一项； 3、年龄在40~80岁； 4、意识清楚、语言交流无障碍并愿意配合本次研究者。

Inclusion criteria

1. According to the diagnostic criteria for COPD in the 2024 edition of GOLD (Global Initiative for Chronic Obstructive Lung Disease), and the condition is in an acute exacerbation phase; 2. Meets any one of the following criteria from the 2022 EXODUS (EXpert consensus On Diaphragm Ultra Sonography in the critically ill) guidelines: diaphragm mobility <2 cm, diaphragm thickness <2.2 mm, or diaphragm thickening fraction <20%; 3 Age between 40 and 80 years; 4.Be conscious, able to communicate verbally without difficulty, and willing to cooperate with the study.

排除标准：

1、存在运动禁忌症：未控制的高血压（＞180/110 mmHg）与低血压（＜90/60 mmHg）；严重的心血管疾病；合并有支气管哮喘急性发作、肺栓塞、气胸等；存在影响运动神经肌肉疾病、骨关节病变等。 2、胸部畸形，已知的膈肌麻痹或慢性神经肌肉疾病； 3、研究过程中依从性差，不愿配合完成相关项目检查与干预。

Exclusion criteria：

1.Existence of contraindications for exercise: uncontrolled hypertension (>180/110 mmHg) and hypotension (<90/60 mmHg); severe cardiovascular disease; concomitant acute exacerbation of bronchial asthma, pulmonary embolism, pneumothorax, etc.; presence of conditions affecting neuromuscular diseases, osteoarthritis, etc. 2. Chest deformities, known diaphragmatic paralysis, or chronic neuromuscular diseases; 3. Poor compliance during the study, unwillingness to cooperate with relevant examinations and interventions.

采用前瞻性研究方法，首先根据表1的分级标准，对AECOPD患者进行仔细评估，并明确将其分为I级、II级和III级三个不同的级别。再使用随机数字表法，随机过程不受任何人的干预，将每一级别的患者随机分为研究组（A组）和对照组（B组）。本研究使用单盲法，确保患者不知道自己被分配到研究组还是对照组。根据患者的分级结果，将A组和B组内的患者分别细分为IA、IIA、IIIA和IB、IIB、IIIB六个亚组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Using a prospective research method, first, based on the grading criteria in Table 1, a detailed assessment was conducted on patients with AECOPD, and they were clearly classified into three different levels: I, II, and III. Then, using the random number table method, a random process was carried out without any intervention from anyone, and patients of each level were randomly divided into the study group (Group A) and the control group (Group B). This study employed a single-blind method to ensure that patients did not know which group they were assigned to (the study group or the control group). Based on the grading results of the patients, the patients in Group A and Group B were further divided into six subgroups: IA, IIA, IIAA, IIB, IIBA, and IIBIA.
