研究目的：

1.构建心血管病高危人群健康行为环境匹配干预方案； 2.验证心血管病高危人群健康行为环境匹配干预方案的有效性和可行性。  

Objectives of Study：

1. To develop an intervention program that aligns health behavior strategies with the environmental context of individuals at high risk for cardiovascular disease; 2. To evaluate the effectiveness and feasibility of the environment-matched health behavior intervention among high-risk populations for cardiovascular disease.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥18岁； ②经检查符合《中国心血管病一级预防指南》标准的心血管病高危人群； ③在目前居住地居住超过1年的常住人口； ④可以阅读并填写调查问卷，沟通交流无障碍； ⑤拥有智能手机，可以使用或经指导后使用微信小程序； ⑥自愿参与本研究并签署知情同意书。

Inclusion criteria

1. Aged >=18 years; 2. Identified as high-risk individuals for cardiovascular disease according to the criteria outlined in the "Chinese Guidelines for the Primary Prevention of Cardiovascular Disease"; 3. Permanent residents who have lived in their current place of residence for more than one year; 4. Able to read and complete questionnaires, with no communication barriers; 5. Possess a smartphone and are able to use, or can use after instruction, the WeChat mini program; 6. Voluntarily agree to participate in the study and provide written informed consent.

排除标准：

1.患有心血管病者； 2.存在严重的躯体、精神心理疾病或认知功能障碍； 3.孕妇或哺乳期妇女； 4.目前正在参与其他教育、心理、行为或药物等类型的干预研究； 5.由于其他原因导致身体活动受限者。

Exclusion criteria：

1. Diagnosed with cardiovascular disease; 2. Presence of severe physical illness, mental disorders, or cognitive impairment; 3. Pregnant or breastfeeding women; 4. Currently participating in other intervention studies involving education, psychological, behavioral, or pharmacological approaches; 5. Individuals with physical limitations that restrict their ability to engage in physical activity for other reasons.
