研究目的：

本研究的目的为观察盆底肌联合肺功能训练对老年股骨粗隆间骨折术后患者肺功能的疗效。通过本研究可为股骨粗隆间骨折术后病人提供包括盆底康复在内的整体康复治疗方案，增强患者肺功能、康复训练耐力，促进患者咳嗽能力及痰液有效排出，降低患者骨折术后肺部感染等并发症的发生风险，改善患者呼吸模式，缩短患者住院时间，提高患者回归家庭后的自理能力，提高患者远期生活质量和社会参与能力。  

Objectives of Study：

The objective of this study was to observe the effect of pelvic floor muscle combined with lung function training on lung function in elderly patients with intertrochanteric fracture. Through this study, the overall rehabilitation treatment plan including pelvic floor rehabilitation can be provided for patients after intertrochanteric fracture of femur, which can enhance lung function and rehabilitation training endurance of patients, promote patients' coughing ability and effective sputum discharge, reduce the risk of complications such as lung infection after fracture surgery, improve patients' breathing pattern, and shorten patients' hospital stay. Improve patients' self-care ability after returning to the family, improve patients' long-term quality of life and social participation ability.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合2015年中国骨质疏松性骨折诊疗指南中髋部骨折诊断标准，经CT/X 线片检查确诊股骨粗隆间骨折且行髓内钉手术治疗； 2.65<=年龄<=80岁，病情稳定，无严重认知障碍（MMSE评分: 文盲>17分，小学>20分，初中及以上>24分）； 3.首次接受髓内钉手术，1周<=术后<=1个月； 4.盆底肌肌力>=Ⅱ级； 5.患者及家属愿意接受盆底肌康复治疗，并签署知情同意书。

Inclusion criteria

1. Meet the diagnostic criteria for hip fracture in the 2015 Chinese guidelines for the diagnosis and treatment of osteoporotic fractures, and confirm the intertrochanteric fracture of the femur by CT/X-ray examination and undergo intramedullary nail surgery; 2.65<=age<=80 years old, stable condition, no severe cognitive impairment (MMSE score: illiteracy> 17 points, primary school > 20 points, junior high school and above> 24 points); 3. Undergo intramedullary nail surgery for the first time, 1 week < = < = 1 month after surgery; 4. Pelvic floor muscle strength >=grade II.; 5. Patients and their families are willing to undergo pelvic floor muscle rehabilitation treatment and sign the informed consent form.

排除标准：

1.合并四肢感染、肿瘤等疾病患者; 2.合并严重心脑血管疾病（如心肌梗死、心律失常、心力衰竭、脑卒中等）或其他脏器类损伤患者; 3.合并严重认知障碍、沟通障碍患者; 4.接受过直肠、结肠手术等不能充分配合进行盆底肌肌力测试和训练的患者.

Exclusion criteria：

1. Patients with limb infection, tumor and other diseases; 2. Patients with severe cardiovascular and cerebrovascular diseases (such as myocardial infarction, arrhythmia, heart failure, stroke, etc.) or other organ injuries; 3. Patients with severe cognitive impairment and communication disorders; 4. Patients who have undergone rectal and colon surgery and cannot fully cooperate with pelvic floor muscle strength testing and training.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据管理采用纸质+电子管理流程，制定电子版CRF表，每位受试者一份，并打印出来，存放于指定位置此表主要内容包括受试者基本信息、临床相关资料、知情同意书、随访记录、副作用量表等。测试的相关数据则录入指定电脑，采用电子化管理流程，进行严格的数据管理与质量控制。 当患者剔除、脱落后，研究者应尽可能与患者联系，完成所能完成的评估项目，并填写试验结束表，详细记录试验治疗记录。对因不良反应而脱落者，经随访最后判断与试验有关者，必须记录在CRF表并通知申办者。对任何脱落病例，研究者必须在CRF表中填写脱落的原因，一般情况下有6种：即不良事件、缺乏疗效、违背试验方案（包括依从性差者）、失访（包括患者自行退出）、被申办者中止和其他。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Each subject had one copy of the electronic CRF form, which was printed out and stored in the designated place. The main contents of the form included the basic information of the subjects, clinical related data, informed consent, follow-up records, side effects scale, etc. The relevant data of the test is entered into the designated computer, and the electronic management process is adopted to carry out strict data management and quality control. When a patient was excluded or dropped out, the investigator should contact the patient as much as possible to complete the evaluation items that could be completed, fill in the end of the trial form, and record the trial treatment record in detail. Dropouts due to adverse events should be recorded on the CRF form and notified to the sponsor. For any dropouts, investigators were required to fill in the CRF form the reasons for dropouts, which generally included six reasons: adverse events, lack of efficacy, protocol violations (including nonadherence), loss to follow-up (including withdrawal by the patient), discontinuation by the sponsor, and others.
