研究目的：

拟开展一项前瞻队列研究，纳入心脑血管疾病、慢性肾脏疾病的高危个体，在基线时采集临床资料和眼底影像数据，随访收集心脑血管及肾功能不全事件的发生情况。基于基线数据，训练机器学习算法，探索眼底影像数据与心脑血管疾病、慢性肾脏疾病诊断的关联关系，构建基于眼底影像数据的心脑血管疾病、慢性肾脏疾病的筛查工具。基于随访数据，探索眼底影像数据与心脑血管事件及肾功能不全发生的关联关系，构建基于眼底影像数据的心脑血管疾病、慢性肾脏疾病发病的预测工具。基于此研究经验，搭建眼与全身病的数据平台，构建眼对全身疾病的筛查预测工具。  

Objectives of Study：

We plan to conduct a prospective cohort study, including individuals at high risk of cardiovascular and cerebrovascular diseases as well as chronic kidney diseases. At the baseline, clinical data and fundus imaging data will be collected. During the follow-up period, the occurrence of cardiovascular and cerebrovascular as well as renal insufficiency events will be collected. Based on the baseline data, machine learning algorithms will be trained to explore the correlation between fundus imaging data and the diagnosis of cardiovascular and cerebrovascular diseases and chronic kidney diseases. A screening tool for cardiovascular and cerebrovascular diseases and chronic kidney diseases based on fundus imaging data will be constructed. Based on the follow-up data, the association between fundus imaging data and the occurrence of cardiovascular events and renal insufficiency will be explored, and a prediction tool for the onset of cardiovascular and cerebrovascular diseases and chronic kidney diseases based on fundus imaging data will be constructed. Based on this research experience, a data platform for eyes and systemic diseases will be established, and a screening and prediction tool for eyes on systemic diseases will be constructed.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.明确诊断的冠心病，包括既往明确诊断的心肌梗死，曾接受冠脉介入治疗或冠状动脉旁路移植术治疗，冠状动脉狭窄>=50%，或胸痛且有心肌缺血的客观依据（负荷心电图或负荷影像检查提示心肌缺血）。
2.明确诊断的脑卒中。
3.不符合1.或者2.，但符合心脑血管病高危个体，定义为合并以下危险因素中的至少 2 种:（1） >=60 岁男性，或>=65 岁女性；（2）糖尿病；（3）高血压；（4）总胆固醇＞5.2 mmol/L，或 LDL-C＞3.4 mmol/L，或 HDL-C＜1.0 mmol/L；(5）BMI>=28 kg/m2；（6）吸烟，定义为目前每天吸烟且持续 1 年及以上；
4.慢性肾脏疾病（CKD）需满足以下任意一项且持续时间超过3个月：（1）肾脏损伤标志，出现以下任意一项：1） 白蛋白尿：尿白蛋白排泄率（UAER）>=30 mg/24 h 或尿白蛋白/肌酐比值（UACR）>=30 mg/g。 2） 尿沉渣异常。 3） 肾小管相关病变。 4） 肾脏组织学异常。 5） 肾脏影像学检查发现结构异常。 6） 肾移植病史。 （2）肾小球滤过率（GFR）下降 1） 估算肾小球滤过率（eGFR）<60 ml/min/1.73m2。 2） 如果肾损伤或肾功能异常持续时间不明确，通过临床评估诊断为CKD。

Inclusion criteria

1.Clearly diagnosed coronary heart disease, including previously clearly diagnosed myocardial infarction, having received coronary intervention therapy or coronary artery bypass grafting treatment, coronary artery stenosis>=50%, or chest pain with objective evidence of myocardial ischemia (stress electrocardiogram or stress imaging examination suggesting myocardial ischemia).
2.A clearly diagnosed stroke.
3.Individuals who do not meet either 1. or 2. but are at high risk of cardiovascular and cerebrovascular diseases are defined as having at least two of the following risk factors: （1） Male aged 60 or above, or female aged 65 or above; （2） Diabetes （3） Hypertension （4）Total cholesterol > 5.2 mmol/L, or LDL-C > 3.4 mmol/L, or HDL-C < 1.0 mmol/L;（5） BMI>=28 kg/m2;（6）Smoking, defined as currently smoking every day for one year or more;
4.Chronic kidney disease (CKD) must meet any one of the following conditions and last for more than 3 months: （1） Signs of kidney injury, presenting any of the following: 1） Albuminuria: Urinary albumin excretion rate (UAER)>=30 mg/24 h or urinary albumin/creatinine ratio (UACR) >=30 mg/g. Abnormal urine sediment. Tubule-related lesions. 2） Histological abnormalities of the kidneys. Structural abnormalities were found in renal imaging examinations. 3） History of kidney transplantation. （2）Decreased glomerular filtration rate (GFR) 1） Estimated glomerular filtration rate (eGFR) <60 ml/min/1.73m2 2）If the duration of renal injury or abnormal renal function is unclear, CKD is diagnosed through clinical assessment.

排除标准：

1.因个体自身原因无法采集符合研究要求的眼底影像数据:（1）屈光介质混浊、严重的黄斑水肿、眼底出血、严重的NPDR、视网膜增殖、严重的黄斑前膜牵拉血管变形等影响眼底数据采集者;（2）既往行视网膜激光治疗，或任一眼接受注射治疗，或者玻璃体视网膜手术史；（3）不能配合完成所需的眼底影像数据的采集：如帕金森等；（4）其他导致无法采集符合研究要求的眼底影像数据的情况；
2.罹患其他严重疾病，预期生存期短于一年，如恶性肿瘤晚期等；
3.无法坚持长期随访；
4.其他研究者认为不宜参加研究的情况；

Exclusion criteria：

1. Due to individual's own reasons, they are unable to collect fundus imaging data that meets the research requirements: （1） Those with opacity of refractive media, severe macular edema, fundus hemorrhage, severe non-proliferative diabetic retinopathy (NPDR), retinal proliferation, severe epiretinal membrane pulling on blood vessels and causing deformation, etc., which affect the collection of fundus data（2）Those who have received retinal laser treatment in the past, or have received injection treatment in one eye, or have a history of vitreoretinal surgery（3）Those who cannot cooperate to complete the collection of the required fundus imaging data: such as Parkinson's disease, etc.（4）Other situations that prevent the collection of fundus imaging data that meets the research requirements;
2. Suffering from other serious diseases with an expected survival period shorter than one year, such as advanced malignant tumors, etc.
3. Unable to persist in long-term follow-up;
4. Other situations that researchers consider inappropriate for participation in the study;
