研究目的：

本项目提供多种全谷物主食进行粗细搭配，制定个体化的饮食方案，并联合动态血糖监测管理系统，实现实时、个体化的饮食指导，以提升患者的自我管理能力，改善代谢紊乱情况。  

Objectives of Study：

This project offers a variety of whole grain staple foods for a combination of coarse and fine grains, formulates individualized dietary plans, and combines a dynamic blood glucose monitoring and management system to achieve real-time and individualized dietary guidance, in order to enhance patients' self-management ability and improve metabolic disorders.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1: 确诊为2型糖尿病 2: 具有读写能力 3: 能够使用智能手机； 4. 年龄 40-75 岁

Inclusion criteria

1: Diagnosed with type 2 diabetes; 2: Have the ability to read and write; 3: Be able to use a smart phone; 4. Age 40-75 years

排除标准：

1: 不适应全谷物食物 2: 过分消瘦或皮下脂肪较少 3: 皮肤油脂分泌过旺导致胶带无法固定 4: 对传感器固定胶带过敏 5: 伴有经皮肤、体液传播的传染病患者 6: 有减肥手术史、消化道疾病病史 7: 有心、肝、肾等主要脏器并发症，或合并有其它严重疾病，精神病患者，服用糖皮质激素或其它影响血糖药物

Exclusion criteria：

1: Not adapted to whole grain foods; 2: Excessive thinness or less subcutaneous fat; 3: Excessive sebum secretion on the skin causes the tape to fail to fix; 4: Allergic to the sensor fixing tape; 5: Patients with infectious diseases transmitted through the skin or body fluids; 6: Has a history of bariatric surgery or digestive tract diseases; 7: Patients with complications in major organs such as the heart, liver and kidneys, or those with other serious diseases, mental patients, and those taking glucocorticoids or other drugs that affect blood sugar;
