研究目的：

本项目依托中心慢病管理信息化平台，拟构建一个多模态、多维度的老年特应性皮炎（AD）单病种队列数据库，全方位纳入临床及生物样本数据，并安排定期随访。以此为基础，深度剖析老年 AD 的人口学、共病及血清学等关键临床特征，运用分层、多状态模型分析法，探寻疾病进展里各状态转移规律，明确起病时间类型、非过敏性共病对治疗的影响，力求为优化老年 AD 治疗方案提供有力循证医学证据，以此达成对该疾病的全过程精细化管理。  

Objectives of Study：

This project relies on the center's chronic disease management information platform and aims to establish a multimodal, multidimensional single-disease cohort database for elderly atopic dermatitis (AD), incorporating comprehensive clinical and biological sample data, along with regular follow-ups. Based on this, it will deeply analyze key clinical characteristics of elderly AD, such as demographics, comorbidities, and serology, using stratified multi-state modeling analysis to explore the transition patterns of various states in disease progression, clarify the types of onset timing, and the impact of non-allergic comorbidities on treatment. The goal is to provide strong evidence-based medical support for optimizing treatment plans for elderly AD, achieving refined management of the entire disease process.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.根据中国张氏诊断标准确诊为特应性皮炎的患者。 2.60岁以上，性别不限。 3.疾病持续时间至少为6个月，并且在过去一年内至少有一次中重度复发。 4.SCORAD（Scoring Atopic Dermatitis）指数>=25分，或EASI（Eczema Area and Severity Index）评分>=16分。 5.在过去6个月内接受过系统治疗或外用激素治疗，但疗效不理想或病情反复者。 6.患者或其法定监护人同意参加研究，并签署知情同意书。

Inclusion criteria

1.Patients diagnosed with atopic dermatitis according to the Zhang's diagnostic criteria in China. 2.Aged 60 and above, regardless of gender. 3.The duration of the illness is at least 6 months, and there has been at least one moderate to severe relapse in the past year. 4.Scoring Atopic Dermatitis(Scorad)>=25 points, or Eczema Area and Severity Index(EASI)>=16 points. 5.Those who have received systemic treatment or topical hormone therapy in the past six months, but with unsatisfactory results or recurrent conditions. 6.The patient or their legal guardian agrees to participate in the study and signs the informed consent form.

排除标准：

1.其他皮肤疾病：同时患有其他可导致类似皮肤病变的皮肤病，如银屑病、皮肤T细胞淋巴瘤等。 2.严重全身性疾病：患有影响免疫功能的严重全身性疾病，如未控制的糖尿病、慢性肾病、肝病或恶性肿瘤。 3.免疫抑制治疗：在过去6个月内接受过免疫抑制剂或生物制剂治疗的患者（除非这些治疗是针对AD）。 4.依从性差：不愿意或不能按时完成随访的患者，或者存在影响长期依从性的精神或行为障碍。 5.严重的感染性疾病：如活动性结核、HIV感染、活动性病毒性肝炎等 6.参与其他临床试验：在过去6个月内参加过其他可能影响本研究结果的临床试验。

Exclusion criteria：

1.Other skin diseases: simultaneously suffering from other skin diseases that can lead to similar skin lesions, such as psoriasis, cutaneous T-cell lymphoma, etc. 2.Severe systemic diseases: Suffering from severe systemic diseases that affect immune function, such as uncontrolled diabetes, chronic kidney disease, liver disease, or malignant tumors. 3.Immunosuppressive therapy: Patients who have received immunosuppressants or biological agents in the past 6 months (unless these treatments were for AD). 4.Poor compliance: patients who are unwilling or unable to complete follow-ups on time, or who have mental or behavioral disorders that affect long-term compliance. 5.Severe infectious diseases: such as active tuberculosis, HIV infection, active viral hepatitis, etc. 6.Participated in other clinical trials: Have participated in other clinical trials in the past 6 months that may affect the results of this study.

口服抗炎药（昆明山海棠片、复方甘草酸苷片、雷公藤多甙片） 2片 3/日联合1-3种抗组胺药； 外用糖皮质激素+润肤霜。

Oral anti-inflammatory drugs (Kunming Mountain Begonia tablets, Compound Glycyrrhizin tablets, Tripterygium Glycosides tablets) 2 tablets 3 times a day combined with 1-3 types of antihistamines. Topical corticosteroids + moisturizer.
