研究目的：

本研究旨在通过评估代盐（含75%氯化钠和25%氯化钾）对青海地区穆斯林高血压患者血压的长期影响，同时监测其在斋月这一特殊时期的使用安全性。  

Objectives of Study：

This study aimed to evaluate the long-term effects of salt substitute (containing 75% sodium chloride and 25% potassium chloride) on hypertension among Muslims in Qinghai Province, and to monitor its safety during the special period of Ramadan.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18-75周岁 2.穆斯林人群（民族不限） 3.高血压史（血压控制不良）： 服用降压药后一般人群目标血压：收缩压 >=140mmHg 和/或舒张压 >=90mmHg 用降压药后特殊人群目标血压(如糖尿病、慢性肾病患者)：收缩压 >=130mmHg 和/或舒张压 >=80mmHg 4.基线血压测量要求： 控制欠佳的判定条件：患者已按医嘱规律服用降压药物，生活方式干预措施落实到位，连续3次以上测量血压仍高于目标值（两次测量间隔至少24小时） 测量时应使用标准测量方法，避免受到情绪、活动等因素影响，可进行家庭血压监测 5.通讯要求：参与者本人或家庭成员能完成互联网调研（包括电话及问卷）

Inclusion criteria

1. Age 18-75 years old 2. Muslims (regardless of ethnicity) 3. History of hypertension (poor blood pressure control): Target blood pressure in the general population after taking antihypertensive drugs: systolic blood pressure >=140mmHg and/or diastolic blood pressure >=90mmHg Target blood pressure in special populations after antihypertensive drugs (e.g., patients with diabetes mellitus, chronic kidney disease): systolic blood pressure >=130mmHg and/or diastolic blood pressure >=80mmHg 4. Baseline blood pressure measurement requirements: Criteria for poor control: the patient has taken antihypertensive drugs regularly as prescribed, lifestyle interventions are in place, and blood pressure is still above the target value after more than 3 consecutive measurements (at least 24 hours between the two measurements) Standard measurement methods should be used to avoid being affected by factors such as mood and activity, and home blood pressure monitoring can be carried out 5. Communication requirements: Participants or their family members can complete the Internet survey (including telephone and questionnaire)

排除标准：

1.继发性高血压、高血压（亚）急症（血压超过180/110mmHg） 2.严重心衰（LVEF<50%）、心源性休克、急性心肌梗死 3.严重肝、肾功能不全（ALT/AST>3倍正常值上限，eGFR<60mlL/kg/1.73m2） 4.参与者或家庭成员正在使用保钾利尿剂或钾补充剂 5.参与者或家庭成员已知有严重肾功能不全 6.参与者或家庭成员有其他使用盐替代品的禁忌症 7.基线血钾≥5.0 mmol/L； 8.恶性肿瘤患者 9.预期寿命不足1年 10.依从性差 11.参与者大多在外就餐（每周外出就餐≤1次）

Exclusion criteria：

1. Secondary hypertension, hypertensive (sub) emergency (blood pressure over 180/110mmHg) 2. Severe heart failure (LVEF<50%), cardiogenic shock, acute myocardial infarction 3. Severe liver and kidney dysfunction (ALT/AST>3 times the upper limit of normal, eGFR<60mlL/kg/1.73m2) 4. Participants or family members are using potassium-sparing diuretics or potassium supplements 5. Participants or family members are known to have severe renal dysfunction 6. Participants or family members have other contraindications to the use of salt substitutes 7. Baseline blood potassium ≥5.0 mmol/L; 8. Patients with malignant tumors 9. Life expectancy is less than 1 year 10. Poor compliance 11. Participants mostly eat out (dining out ≤1 time per week)
