研究目的：

比较Exparel?局部浸润 vs 神经阻滞对骨折患者术后72小时活动痛VAS评分，为骨科患者术后镇痛提供更优方案  

Objectives of Study：

Compare Exparel? local infiltration versus nerve block for postoperative activity pain VAS scores at 72 hours in patients with fractures, providing a superior analgesic regimen for postoperative pain management in fracture patients.

纳入标准：

同意实施此研究计划的 年龄 ：18-85岁 手术类型：单侧闭合性下肢骨折有限切开复位内固定术（掌骨/内外踝/胫骨） ASA分级 : I-III级

Inclusion criteria

Consent for implementing this research plan: Age: 18-85 years old; Surgical type: Single-side closed lower limb fracture limited open reduction and internal fixation (metatarsal bone/tibial malleolus/tibia); ASA classification: Grade I-III

排除标准：

拒绝实施此研究计划的 开放性骨折或合并神经血管损伤 酰胺类局麻药过敏史 肝功能异常（ALT/AST > 2倍上限） 长期使用阿片类药物（>30mg吗啡当量/日） 认知障碍或语言沟通障碍 合并严重呼吸循环等内科疾病

Exclusion criteria：

Refusal to implement this research plan due to: open fractures or combined neurovascular injuries, history of allergy to amide local anesthetics, abnormal liver function (ALT/AST > 2 times the upper limit), long-term use of opioids (>30 mg morphine equivalent per day), cognitive impairment or communication disorders, and severe internal medical conditions such as respiratory and circulatory diseases.
