研究目的：

1.对比氯己定漱口、口香糖机械清洁及其联合应用对DLT插管患者口腔生物膜清除效能及下呼吸道菌群迁移的影响； 2.分析口腔微生物组动态变化与术后炎症指标（CRP、IL-6、PCT）及全身炎症反应评分等的关联性； 3.明确不同干预措施对术后呼吸系统并发症（肺炎、肺不张）的预防效应阈值，建立循证医学支持的围术期口腔管理方案。  

Objectives of Study：

1. Compare the efficacy of chlorhexidine mouthwash, chewing gum mechanical cleaning, and their combined application on the removal of oral biofilm and the migration of lower respiratory tract microbiota in patients undergoing DLT intubation; 2. Analyze the association between dynamic changes in the oral microbiome and postoperative inflammatory markers (CRP, IL-6, PCT) and systemic inflammatory response scores; 3. Determine the threshold for the preventive effects of different intervention measures on postoperative respiratory complications (pneumonia, atelectasis) and establish an evidence-based perioperative oral management protocol.

纳入标准：

ASA分级I-II级；年龄18-75岁

Inclusion criteria

ASA classification I-II; age 18-75 years.

排除标准：

急症手术患者；合并口腔及牙周疾病者；合并严重肝肾功能不全、心力衰竭等严重疾病者；合并术前肺部感染及损伤者；合并新近肋骨骨折者；合并脑外伤者；精神障碍不能配合调查者；已知对氯已定过敏者； <10颗牙齿者；最近3个月内使用免疫抑制药物或抗生素者；肺功能检查严重异常者（FVC<预计值的40%，FEV1<预计值的30%，MVV低于预计值的50%等）；术前低氧血症患者

Exclusion criteria：

Patients undergoing emergency surgery; patients with concomitant oral and periodontal diseases; patients with severe liver or kidney dysfunction, heart failure, or other serious conditions; patients with preoperative pulmonary infections or injuries; patients with recent rib fractures; patients with head injuries; patients with mental disorders unable to cooperate with the investigation; patients with known allergies to chlorhexidine; Patients with fewer than 10 teeth; patients who have used immunosuppressive drugs or antibiotics within the past 3 months; patients with severe abnormalities in pulmonary function tests (FVC < 40% of predicted value, FEV1 < 30% of predicted value, MVV < 50% of predicted value, etc.); patients with preoperative hypoxemia.

所有患者的术前口腔清洁工作在麻醉准备间由一名单独的工作人员完成，后续的麻醉管理、标本采集、细菌计数、术后随访由不知道分组的研究人员完成。

Blinding：

Preoperative oral cleaning for all patients was performed by a single staff member in the anesthesia preparation room. Subsequent anesthesia management, specimen collection, bacterial counting, and postoperative follow-up were performed by researchers who were unaware of the groupings.
