研究目的：

构建基于多模态融合预测模型，以实现精准预测术后肝功能衰竭，指导临床决策。  

Objectives of Study：

A multimodal fusion prediction model was developed to enhance the accuracy of post-hepatectomy liver failure prediction and provide evidence-based support for clinical decision-making.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.自愿受试，依从性好，配合随访；
2.年龄大于等于18岁，小于等于80岁；
3.经影像学检查证实存在肝占位，并接受肝切除术的患者；
4.术前同时接受了腹部增强CT扫描及Gd-EOB-DTPA增强MRI扫描，且扫描时间均与手术时间间隔3个月内；
5.术前2周内有完整的生化及临床信息，包括年龄、性别、糖尿病史、手术记录、血肌酐、尿素、血清总胆红素、凝血酶原时间国际标准化比值等检测结果；
6.术后第5天至30天有血清总胆红素、凝血酶原时间国际标准化比值检测结果用于诊断是否发生术后肝衰竭。

Inclusion criteria

1.Voluntary participation, demonstrating good compliance, and willingness to cooperate with follow-up procedures.
2.Age: Participants must be at least 18 years of age and not older than 80 years.
3.The liver mass was comfirmed through imaging examinations, and hepatectomy was subsequently performed.
4.All patients received both contrast-enhanced abdominal CT scans and Gd-EOB-DTPA-enhanced MRI scans prior to surgery, with all imaging performed within three months of the operative date.
5.There was complete biochemical and clinical information available within two weeks prior to surgery, including informations of age, sex/gender, history of diabetes, surgical records, serum creatinine, urea, total serum bilirubin, and the international normalized ratio (INR) of prothrombin time.
6.From the 5th to the 30th day post-operation, INR and total serum bilirubin levels were monitored to assess the occurrence of postoperative liver failure.

排除标准：

1.肝脏增强CT和/或MR图像存在明显伪影；
2.增强CT和/或MR检查存在序列缺失；
3.缺乏完整的临床病史和实验室检测结果；
4.术前曾接受过抗肿瘤治疗（包括经动脉化疗栓塞、系统性抗肿瘤治疗）；
5.联合肝脏分割和门静脉结扎的分阶段肝切除术。

Exclusion criteria：

1.Noticeable artifacts were observed in the enhanced CT and/or MR images of the liver.
2.Imaging sequences were missing from the enhanced CT and/or MR examinations.
3.The clinical history was incomplete, and laboratory test results were unavailable.
4.Prior administration of anti-tumor treatments before surgery, including transarterial chemoembolization and systemic chemotherapy.
5.History of associating ?liver partition and portal vein ligation for staged hepatectomy (ALPPS).

本项目采用“ISGLS标准”，作为本项目中术后肝功能衰竭的诊断标准及分级标准。诊断标准为术后第5天和之后出现的凝血酶原时间国际标准化比值（PT-INR）异常升高和高总胆红素血症。

This project utilizes the "ISGLS criteria" as the diagnostic and grading standards for postoperative liver failure. The diagnostic criteria consist of abnormally elevated prothrombin time international normalized ratio (PT-INR) and hyperbilirubinemia occurring on or after the fifth postoperative day
