研究目的:

1).明确LSG 手术对肥胖患者肠道微生物群组成的影响:通过对比LSG 手术前后的微生物群数据,明确手术对微生物群多样性和丰度的影响。 2). 论证肠道微生物群变化与代谢改善之间的因果关系:通过统计分析,论证LSG手术后肠道微生物群的变化与患者代谢指标改善之间的相关性。 3). 阐明特定微生物群在宿主代谢调节中的作用:通过动物实验,阐明特定有益菌群在LSG手术后对宿主代谢的调节作用及其机制。 4). 探讨LSG手术对肠道屏障功能的影响及其机制:通过评估肠道通透性和相关生物标志物,探讨LSG手术对肠道屏障功能的影响及其与微生物群变化的关系。  

Objectives of Study:

1. Determine the impact of LSG on gut microbiota composition in obese patients: Compare pre- and post-operative microbiota data to assess changes in microbial diversity and abundance. 2. Establish causality between gut microbiota alterations and metabolic improvement: Perform statistical analyses to correlate post-LSG microbiota shifts with improvements in metabolic parameters. 3.Elucidate the role of specific microbiota in host metabolic regulation: Conduct animal studies to clarify the mechanisms by which beneficial bacterial taxa modulate host metabolism after LSG. 4. Investigate LSG-induced effects on gut barrier function and underlying mechanisms: Evaluate intestinal permeability and related biomarkers to explore the relationship between LSG, gut barrier integrity, and microbiota changes.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.基于 WHO 标准(BMI ≥30 kg/m 2)或结合腰围/体脂率(如亚洲人群 BMI ≥27.5kg/m2);
18-65 岁;
2.明确肥胖持续时间(如≥6 个月),避免偶发性体重波动干扰;
3.HbA1c <6.5%(排除糖尿病);
4.行为与药物控制:1)饮食与运动:要求受试者保持稳定生活方式(如近 3 个月无剧 烈饮食调整或运动计划变更)。2)药物使用:允许纳入未使用影响代谢药物 (如降糖药、减肥药)的受试者,但需详细记录。

Inclusion criteria

1.Based on WHO criteria (BMI ▍>=30 kg/m^2) or combined with waist circumference/body fat percentage (e.g., BMI >=27.5 kg/m^2 for Asian populations). 2.Aged 18–65 years; 3.Confirm obesity duration (e.g., >=6 months) to exclude transient weight fluctuations. 4.Behavioral and Pharmacological Controls: Diet/Exercise: Participants must maintain stable lifestyle (e.g., no significant dietary/exercise changes in prior 3 months). Medications: Eligible if no use of metabolism-affecting drugs (e.g., hypoglycemics, anti-obesity agents); detailed documentation required.

排除标准:

1.干扰肠道菌群的医疗因素:1)药物使用:近3个月内使用 抗生素、质子泵抑制剂(PPI)、免疫抑制剂或系统性激素;近 1 个月内 使用益生菌/益生元。2)胃肠道疾病:炎症性肠病(IBD)、肠易激综合征(IBS)、胃肠道手术史(如胃旁路术)。3)全身性疾病:自身免疫病、恶性肿瘤、慢性肝/肾功能不全。
2.代谢与内分泌混杂因素:甲状腺功能异常(如TSH超出正常范围)、库欣综合征等内分泌疾病。
3.行为与生理干扰:1)近期体重变化:3 个月内体重波动≥5%(可能提示非稳定性肥胖)。2)特殊饮食:极低碳水化合物饮食、生酮饮食、高剂量膳食补充剂使用者。3)吸烟/饮酒:重度饮酒(男性>40g/天,女性>20g/天)或吸烟(>10 支/天)。
4.无法配合粪便采样(如严重便秘/腹泻)、近期接受肠道准备(如肠镜检查后1个月内)。

Exclusion criteria:

1.Gut microbiota modifiers: Medications: Antibiotics, PPIs, immunosuppressants, or systemic hormones within 3 months; probiotics/prebiotics within 1 month. GI disorders: IBD, IBS, or prior GI surgery (e.g., gastric bypass). Systemic diseases: Autoimmune disorders, malignancies, chronic hepatic/renal insufficiency. 2.Metabolic/endocrine confounders: Thyroid dysfunction (e.g., abnormal TSH levels), Cushing’s syndrome, or other endocrine disorders. 3.Behavioral/Physiological Exclusions: Recent weight fluctuation: >=5% change within 3 months (indicating non-stable obesity). Special diets: Ketogenic, very-low-carbohydrate, or high-dose supplement use. Substance use: Heavy alcohol (>40g/day [male]/>20g/day [female]) or smoking (>10 cigarettes/day). 4.Inability to provide fecal samples (e.g., severe constipation/diarrhea) Recent bowel preparation (e.g., within 1 month post-colonoscopy);

数据录入:所有研究数据(包括临床数据、实验室检测结果、测序数据等)将由专人负责录入电子数据库(Excel),采用双人独立录入模式以确保准确性。数据录入时需标注时间、样本编号及受试者唯一识别码,避免个人信息泄露。 数据核查:定期(每3个月)对数据进行逻辑核查与一致性检查,发现异常值时需追溯原始记录并修正。 关键指标(如BMI、微生物群多样性指数)需由两名研究人员交叉验证。 数据存档:原始数据(电子版与纸质版)保存于南宁市第二人民医院数据中心,加密存储,访问权限仅限于研究团队成员。生物样本数据(如DNA序列)备份至云端服务器(如NCBI数据库),脱敏处理后仅保留研究编号。研究结束后,数据存档至少10年,销毁需经伦理委员会批准。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data Management Protocol Data Entry All research data (clinical, laboratory, sequencing) will be entered into electronic databases (Excel) by trained personnel using dual-independent entry to ensure accuracy. Entries must include timestamp, sample ID, and anonymized subject identifier (no personal information). Data Verification Quarterly logic/consistency checks with source documentation review for outliers. Key metrics (BMI, microbial diversity indices) require cross-validation by two researchers. Data Archiving Primary data (electronic/paper) stored in secured servers at Nanning Second People's Hospital with role-based access. Biomolecular data (e.g., DNA sequences) backed up to cloud platforms (e.g., NCBI) under anonymized study IDs. Retention: >=10 years post-study. Destruction requires ethics committee approval.
