研究目的：

第一步，根据回顾性临床资料建立急诊气管插管后循环崩溃的风险预测列线图，评估气管插管后低血压与预后的关系。第二步，通过前瞻性研究，床旁超声快速检查患者气管插管前的中心静脉直径和塌陷率，分析它们对气管插管后低血压预测价值，建立和评价基于超声联合临床的气管插管后低血压预测模型。  

Objectives of Study：

In the first stage, a predictive nomogram model of circulatory collapse after emergency tracheal intubation was created based on retrospective clinical data. To assess the relationship between post-tracheal intubation hypotension and 28-day mortality in patients. In the second stage, bedside ultrasound was used to rapidly examine the central venous diameter and collapse rate of patients before tracheal intubation by prospective observation. Their predictive value for post-tracheal intubation hypotension was analyzed, and an ultrasound-based combined clinical post-tracheal intubation hypotension prediction model was established and evaluated.

床旁超声快速检查患者气管插管前的中心静脉（下腔静脉，右侧颈静脉，右侧锁骨下静脉）直径和塌陷率 

Description for medicine or protocol of treatment in detail：

Rapid bedside ultrasound examination of central venous (inferior vena cava, right jugular vein, right subclavian vein) diameters and collapse rates in patients before tracheal intubation 

纳入标准：

①符合气管插管适应证，并在我院急诊室予经口气管插管；②年龄≥18岁；③同意参加本研究。

Inclusion criteria

1.Patients undergoing initial oral endotracheal intubation in the emergency department. 2.Age threshold of >= 18 years. 3 Agree to participate in this study.

排除标准：

①气管插管前60分钟内出现收缩压(SBP)<90 mm Hg或平均动脉压(MAP)<65 mm Hg；②气管插管前已使用血管活性药物；③本次气管插管为48小时内再次气管插管。

Exclusion criteria：

1.Systolic blood pressure (SBP) <90 mm Hg or mean arterial pressure (MAP) <65 mm Hg occurred within 60 minutes prior to tracheal intubation; 2.vasoactive drugs had been used prior to tracheal intubation; 3. the current tracheal intubation was a repeat tracheal intubation within 48 hours.

在气管插管前请课题组医师使用迈瑞M9超声机行床旁超声检查，患者取0°仰卧位，该医师对患者急诊诊疗不知情。通过彩色血流多普勒和可压缩性来识别相关静脉，当整个静脉的周长可见时，就进行了测量，注意不要施加压力,可能会压迫静脉。行右颈内静脉（IJV）检查，将线阵超声探头放置于颈部右侧横切面，位于胸锁骨关节上方2cm处，记录3个呼吸周期IJV的最大、最小直径，由此得出IJVCI（最大直径-最小直径/最大直径×100%），取平均值。右侧锁骨下静脉（SCV）检查，使用相控阵探头，将探头置于锁骨下方，锁骨中外三分之一处，获得SCV的短轴视图，在3个呼吸周期中测量最大和最小直径，并计算并导出SCVCI。下腔静脉（IVC）检查，使用相控阵探头，将探头纵置于剑突下区域，在IVC距离右心房开口2 cm处旋转90°，测量IVC横截面，以避免呼吸导致位移误差，在3个呼吸周期中测量最大和最小直径，并计算并导出IVCCI。

①患者人口统计学数据、合并症、急诊主诊断、气管插管原因（呼吸衰竭，心力衰竭，昏迷，其他），插管前1小时内补液量（<250ml，250-500ml，>500ml）。入急诊至气管插管的时间，诱导插管前的患者生命体征（体温、血压、心率、呼吸频率和经皮氧饱和度），休克指数，改良休克指数，格拉斯哥昏迷评分、白细胞水平、红细胞压积、血清钠离子浓度、钾离子浓度、肌酐水平、白蛋白水平、动脉血PH、乳酸水平、碳酸氢根浓度、氧分压、二氧化碳分压、氧合指数。②气管插管医生的职称（住院医师，主治医师，副主任医师或主任医师），诱导药物，插管次数。患者呼吸机通气设置，插管后1小时内首次出现低血压时间，PIH定义为平均动脉压(MAP)<65 mm Hg或使用血管活性药物。
