研究目的：

1.构建老年2型糖尿病患者肌少症相关知信行调查问卷、实施基于健康信念模式的健康教育方案，为其他慢性病患者健康教育方案的制定提供借鉴。 2.探索基于健康信念模式的老年2型糖尿病伴肌少症健康教育方案对该类患者的干预效果，为其他相关研究提供参考。  

Objectives of Study：

1. to construct a questionnaire on knowledge, belief and behavior related to sarcopenia in elderly patients with type 2 diabetes mellitus and a health education program based on the health belief model, to provide reference for the development of health education programs for patients with other chronic diseases. 2.To explore the intervention effect of the health education program based on health belief model for elderly type 2 diabetes with sarcopenia on this kind of patients, and to provide reference for other related studies.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）按照WHO（1999）提出的糖尿病诊断标准确诊为2型糖尿病； （2）根据亚洲肌肉减少症工作组2019年关于肌肉减少症诊断和治疗的共识（AWGS2）确诊为肌少症； （3）血糖控制稳定者； （4）60岁≤年龄≤85岁； （5）意识清楚，活动自如； （6）能够熟练运用智能手机，达到接受在线健康教育需要； （7）自愿参与本研究并签署知情同意书。

Inclusion criteria

1. Diagnosis of type 2 diabetes mellitus confirmed according to the diagnostic criteria for diabetes mellitus proposed by WHO (1999); 2. Confirmed diagnosis of sarcopenia according to the Asian Working Group on Sarcopenia 2019 Consensus on Diagnosis and Treatment of Sarcopenia (AWGS2); 3. Those with stable glycemic control; 4. 60 years <= age <= 85 years; 5.Being conscious and moving freely; 6. Being able to use smartphone skillfully to meet the needs of receiving online health education; 7. Voluntarily participate in this study and sign an informed consent form.

排除标准：

（1）有严重的糖尿病并发症，如肾功能不全、视力障碍、糖尿病足等不能按要求饮食及锻炼者； （2）其他重大疾病，如心脏病、肝脏、脑卒中、血液病、大手术等无法参加研究者； （3）严重胃肠道疾病影响消化、吸收功能者； （4）有视力、听力、言语、阅读、理解力障碍不能配合研究者；

Exclusion criteria：

1. Those with severe diabetic complications such as renal insufficiency, visual impairment, diabetic foot, etc. who are unable to eat and exercise as required; 2. Those with other major diseases, such as heart disease, liver, stroke, blood disease, major surgery, etc. who are unable to participate in the study; 3. Those with severe gastrointestinal diseases affecting digestion and absorption functions; 4. Those with vision, hearing, speech, reading, and comprehension disorders who cannot cooperate with the study;

在对照组微信群中，常规进行糖尿病及肌少症健康教育，同样采用线上微信群教育形式；入组后建立健康档案，进行肌少症健康知识测试等有关资料收集，健康教育的内容主要为自建公众号“肌少特工队”优质科普文章、视频等，如“2型糖尿病伴肌少症的易感性、严重性”主题视频资料与公众号文章；提醒患者规范血糖管理，对于患者提出的疑问积极解答，维持微信群内良好环境，对于对照组患者一对一的问题积极解答，对照组与干预组在同样时间节点（干预前、干预后6周）进行相关资料收集并记录。

The content of the health education was mainly the self-built public number, "sarcopenia team", high-quality popular science articles and videos, such as Video materials and public number articles on the theme of "susceptibility and severity of type 2 diabetes mellitus with sarcopenia"; reminding patients to standardize their blood glucose management, actively answering questions raised by patients, maintaining a good environment in the WeChat group, and actively answering one-on-one questions from patients in the control group, and providing relevant information to the patients in the control group and the intervention group at the same time points (pre-intervention and 6 weeks post-intervention), 6 weeks post-intervention) for relevant data collection and recording.

干预组与对照组均在干预前收集患者个人基线资料，建立健康档案并记录，首次收集时间节点为：在医疗机构内分泌门诊确定符合纳排标准后现场当即进行数据收集并记录，随后遵循随机分组的方法，将患者分别纳入干预组与对照组，建立2个微信群（即对照组群与干预组群）。按照先后纳入的顺序依次进群，当某一组干预人数纳入够样本量（38人）时，即开始本组的群体健康教育干预。 根据开始干预的时间节点不同，评价时间根据每批次实际开始的时间收集数据并评价。在完成共6次的健康教育干预（每周一次，共6周）后的第6周，即入组开始干预后3个月时，再次邀请患者来到内分泌科门诊复诊，并检测患者相关评价指标，对干预效果进行评价。

Both the intervention group and control group collected patients' personal baseline data before the intervention, established health records and records, the first collection time node: in the endocrine outpatient clinic of the medical institution to determine that meets the criteria for natriuretic data collection and records on the spot, and then follow the method of randomized grouping, the patients were included in the intervention group and the control group, and the establishment of two WeChat groups (i.e., the control group group and the intervention group group). The groups were entered sequentially according to the order of inclusion, and when the number of intervention participants in a group was included in a sufficient sample size (38 participants), the group health education intervention in this group was started. Depending on the point in time when the intervention was started, the evaluation time was based on the actual start time of each batch for data collection and evaluation. At the 6th week after completing a total of 6 health education interventions (once a week for 6 weeks), i.e., 3 months after enrollment in the group to start the intervention, the patients were invited again to come to the outpatient clinic of the Department of Endocrinology for follow-up, and the relevant evaluation indexes of the patients were detected to evaluate the effect of the intervention.

1.便利选取在河南中医药大学第一、二附属医院内分泌科门诊就诊的患者。2.方法：使用已经构建的老年2型糖尿病伴肌少症健康教育干预方案，进行为期12周的健康教育干预，将符合纳排标准的研究对象按随机数字表法随机分为2组，一个干预组（基于健康信念模式的健康教育），一个对照组（常规性健康宣教），两组患者各38例。

1.Conveniently selected patients who attended the outpatient clinics of the Department of Endocrinology in the First and Second Affiliated Hospitals of Henan University of Traditional Chinese Medicine.2.Methods: using the already constructed health education intervention program for type 2 diabetes mellitus with sarcopenia in the elderly, a 12-week health education intervention was carried out, and the study subjects who met the criteria of the natatorium were randomly divided into 2 groups according to the randomized numerical table method, an intervention group (health education based on the health belief model), and a control group (routine health education), with 38 patients in each of the two groups.
