研究目的:

中国的臭氧污染及其所导致的疾病负担愈发严峻,热浪等极端天气既是臭氧污染的诱因之一,亦具有直接的健康危害。本研究计划开展针对臭氧和高温暴露的随机交叉对照试验,基于多组学方法(转录组学、蛋白质组学、代谢组学等),厘清臭氧和高温暴露影响人体健康的分子生物学机制,揭示高温与臭氧健康效应的协同机制。  

Objectives of Study:

The burden of diseases caused by ozone pollution in China is becoming increasingly severe. Extreme weather events, such as heatwaves, are not only a trigger for ozone pollution but also pose direct health hazards. This study plans to conduct a randomized crossover trial on ozone and heat exposure. Using multi-omics approaches (including transcriptomics, proteomics, and metabolomics), the study aims to elucidate the molecular biological mechanisms by which ozone and heat exposure affect human health and to reveal the synergistic mechanisms of the health effects of heat and ozone.

纳入标准:

年龄 18岁以上,60岁以下;身体健康;男女比例 1:1。

Inclusion criteria

Participants will be aged 18 to 60 years, in good health, and with a gender ratio of 1:1.

排除标准:

吸烟者(含电子烟)或戒烟少于 3 年者;经医学诊断患有慢性心脑血管疾病(如缺血性心脏病)、呼吸系统疾病(如哮喘或慢性阻塞性肺病)、糖尿病或其他器官或系统功能障碍、会干扰受试者理解和参与研究能力的活动性精神疾病、免疫系统疾病(如 HIV、淋巴瘤、白血病)或存在药物或酒精滥用;BMI >35 或 <18;备孕中、已怀孕或处于哺乳期者;过去 5 年间曾做过重大手术者;无法采血者;无法签署知情同意书者。

Exclusion criteria:

Smokers (including electronic cigarettes) or those who have quit smoking for less than 3 years; individuals medically diagnosed with chronic cardiovascular diseases (such as ischemic heart disease), respiratory diseases (such as asthma or chronic obstructive pulmonary disease), diabetes, or other organ or system dysfunctions; those with active psychiatric disorders that interfere with their ability to understand and participate in the study, immune system diseases (such as HIV, lymphoma, leukemia), or a history of drug or alcohol abuse; individuals with a BMI >35 or <18; those who are planning to conceive, currently pregnant, or breastfeeding; individuals who have undergone major surgery in the past 5 years; those unable to provide blood samples; and individuals unable to sign the informed consent form.

由独立于本研究的统计学专家使用计算机生成一个包含1到48的随机序列,例如,生成的序列可能是:[23, 15, 34, 2, 45, ..., 11]。将随机序列前24个数字分配到22°C室温实验组,后24个数字分配到32°C高温实验组。在22°C室温实验组中,再次随机将24个受试者分为两组,每组12人,在32°C高温实验组中,同样将24个受试者随机分为两组。

Randomization Procedure (please state who generates the random number sequence and by what method):

A random sequence containing the numbers 1 to 48 is generated using a computer by a statistician independent of this study. For example, the generated sequence might be: [23, 15, 34, 2, 45, ..., 11]. The first 24 numbers of the random sequence are assigned to the 22°C room temperature experimental group, and the last 24 numbers are assigned to the 32°C high-temperature experimental group. Within the 22°C room temperature experimental group, the 24 subjects are randomly divided into two groups of 12 each. Similarly, within the 32°C high-temperature experimental group, the 24 subjects are randomly divided into two groups of 12 each.

盲法:

为了最大程度减少偏倚,本研究采用双盲设计(仅针对臭氧,温度易被感知识别,无法做到双盲)。首先,受试者对其具体分组情况不知情,在实验过程中,受试者接受相同的程序和环境条件,无法通过外部因素判断其组别。同样,实验操作人员(包括暴露操作人员和数据收集人员)也对每个受试者的具体分组情况不知情。操作人员仅根据信封中的分组信息进行实验操作,不接触具体的分组信息。数据分析人员在数据分析前对所有数据进行编码处理,使用盲化编号确保分析过程中不受分组信息的影响。

Blinding:

To minimize bias to the greatest extent, this study employs a double-blind design. First, the subjects are unaware of their specific group assignments. During the experiment, subjects undergo the same procedures and environmental conditions, making it impossible for them to determine their group based on external factors. Similarly, the experimental operators (including exposure operators and data collectors) are also unaware of each subject's specific group assignment. The operators conduct the experimental procedures based solely on the grouping information in the envelopes without accessing specific group details. Before data analysis, the data analysts encode all the data, using blinded identifiers to ensure that the group information does not influence the analysis process.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

1) 数据录入:录入采用相应的数据库系统双人双机录入,之后对数据库进行两遍比对。测量的人群数据结果在实验完成后进行录入。 2) 数据核查和管理:当基线及每次随访调查问卷经双份输入并核对无误后,由项目研究人员提供研究进度及基本情况报告,其内容包括研究完成情况(含脱落受试者清单)、入选/排除标准检查、完整性检查、逻辑一致性检查、离群数据检查、时间窗检查、不良事件检查等。 3) 数据存档:研究进度及基本情况报告按编号的顺序归档保存,并填有检索目录等,以备查考。电子数据文件包括数据库、检查程序、分析程序、分析结果、编码本和说明文件等,分类保存,并有多个备份保存于不同磁盘或记录介质上,妥善保存,防止损坏。所有原始档案按相应规定内的期限保存。 4) 实施数据加密和访问控制措施,确保所有受试者的个人数据和生物样本的保密性。本项目将在北京大学伦理委员会审查监督下进行,所收集的信息、实验数据只用于科学研究,不会泄露研究对象的任何个人信息。所采集的样品均以编码表示,不会出现任何能够定位到研究对象的信息,包括姓名,年龄,住址等所有个人信息将会严格保密。测量指标及相关分析结果在发表时,会以人群总体的形式给出,不会显示某位受试者的具体信息。仅国家监管部门、北京大学科研管理部门、伦理委员会有权在指定地点查阅受试者的原始资料,其中可能包括研究对象的个人信息;

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

1) Data Entry: Data entry is conducted using a dual-person, dual-computer system with the corresponding database system. The database is then compared twice for verification. Population measurement data results are entered after the completion of the experiment. 2) Data Verification and Management: Once the baseline and each follow-up survey questionnaires have been double-entered and verified for accuracy, the project researchers will provide a report on the study progress and basic information. This report includes the study completion status (including a list of dropout subjects), inclusion/exclusion criteria checks, completeness checks, logical consistency checks, outlier data checks, time window checks, and adverse event checks. 3) Data Archiving: The study progress and basic information reports are archived in numerical order and include an index for reference. Electronic data files, including the database, verification programs, analysis programs, analysis results, codebooks, and explanatory documents, are saved categorically with multiple backups stored on different disks or media to prevent damage. All original files are preserved according to the specified retention period. 4) Data Encryption and Access Control: Measures are implemented to encrypt data and control access to ensure the confidentiality of all subjects' personal data and biological samples. This project will be conducted under the review and supervision of the Peking University Ethics Committee. The collected information and experimental data will be used solely for scientific research and will not disclose any personal information of the research subjects. The collected samples will be coded, and no information that can identify the research subjects, including names, ages, addresses, or any personal details, will be disclosed. Measurement indicators and related analysis results will be presented in aggregate form when published, without showing specific information of any subject. Only national regulatory authorities, Peking University research management departments, and the ethics committee have the right to review the original data of the subjects at designated locations, which may include personal information of the research subjects.
