本研究旨在通过前瞻性临床实践与技术创新,系统评估基于达芬奇机器人系统的单孔技术(SPRS)在儿童胆总管囊肿根治术中的应用价值,包括: 1.技术可行性验证:探索单孔机器人系统在儿童狭小解剖空间内完成囊肿切除、肝总管空肠吻合等关键步骤的操作可行性,识别技术难点并提出优化策略;评估基于达芬奇Xi系统的单孔技术在儿童胆总管囊肿根治术中的学习曲线; 2.安全性及有效性分析:通过对比传统多孔机器人手术与腹腔镜手术的围手术期指标(如手术时间、出血量、中转开放率)、术后并发症(胆漏、肠梗阻、吻合口狭窄)及长期预后(囊肿复发率、胆道功能),明确SPRS的临床优势与潜在风险; 3.标准化流程构建:结合儿童胆道发育特点,制定单孔机器人手术的规范化操作路径,包括切口定位、器械布局、解剖标志识别及应急方案,为技术推广提供理论依据; 4.综合效益评价:从创伤应激、美容效果、康复速度及心理适应性多维度评估SPRS对患儿生活质量的影响,阐明其在儿科微创外科中的综合应用价值。 通过上述研究,期望为儿童胆总管囊肿的精准微创治疗提供新技术范式,推动机器人单孔手术在儿科复杂胆道疾病中的临床转化。 |

Objectives of Study: |
The aim of this study is to systematically evaluate the application value of Single-Port Robotic Surgery (SPRS) based on da Vinci robotic system in the radical resection of choledochal cyst in children through prospective clinical practice and technological innovation, including: 1. Technical feasibility verification: Explore the operational feasibility of using a single-port robotic system to complete key steps such as cyst resection and hepatic common duct jejunal anastomosis in children's narrow anatomical spaces, identify technical difficulties, and propose optimization strategies; Evaluate the learning curve of single-port technique based on Da Vinci Xi system in the radical resection of choledochal cyst in children; 2. Safety and effectiveness analysis: By comparing the perioperative indicators (such as surgical time, bleeding volume, and transition rate), postoperative complications (bile leakage, intestinal obstruction, anastomotic stenosis), and long-term prognosis (cyst recurrence rate, biliary function) of traditional porous robot surgery and laparoscopic surgery, the clinical advantages and potential risks of SPRS are clarified; 3. Standardized process construction: Based on the developmental characteristics of children's biliary tract, develop a standardized operation path for single-port robotic surgery, including incision positioning, instrument layout, anatomical landmark recognition, and emergency plans, providing theoretical basis for technology promotion. |

纳入标准: |
1.年龄与诊断:0-14岁,经超声、MRI/MRCP或术中胆道造影确诊为Todani I 型或IV 型胆总管囊肿(需手术干预的亚型); 2.手术指征:无急性感染期(如无发热、C反应蛋白正常); 3.解剖条件:无严重腹腔粘连(术前影像学评估)、肝门部解剖结构清晰可辨; 4.家庭配合:监护人同意行机器人手术,签署知情同意书,承诺配合术后随访。 |

Inclusion criteria |
1. Age and diagnosis: 0-14 years old, confirmed by ultrasound, MRI/MRCP or intraoperative cholangiogram with Todani type I or type IV choledochal cyst (subtype requiring surgical intervention); 2. Indications for surgery: no acute infection (such as no fever, normal C-reactive protein); 3. Anatomical conditions: no severe abdominal adhesions (preoperative imaging evaluation), clear and distinguishable anatomical structure of the hilar part of the liver; 4. Family cooperation: The guardian agrees to perform robotic surgery, signs the informed consent form, and promises to cooperate with the postoperative follow-up. |

排除标准: |
1.合并症禁忌:严重心肺功能不全、凝血功能障碍(INR>1.5或血小板<80×10^9/L)或其他机器人手术禁忌证; 2.复杂病理状态:既往上腹部手术史、合并肝内胆管扩张需联合肝叶切除、疑似癌变(影像学提示囊壁增厚伴强化); 3.技术限制:囊肿位置特殊或严重解剖变异(如紧贴门静脉主干,单孔视角难以安全分离); 4.伦理排除:未获得医院伦理委员会批准或监护人拒绝参与研究。 |

Exclusion criteria: |
1. Contraindications for comorbidities: severe cardiopulmonary insufficiency, coagulation dysfunction (INR>1.5 or platelets <80×10^9/L) or other contraindications to robotic surgery; 2. Complex pathological state: history of previous epigastric surgery, combined with intrahepatic bile duct dilation requiring combined hepatic lobectomy, suspected cancerous transformation (imaging showed cyst wall thickening with enhancement); 3. Technical limitations: special location of the cyst or severe anatomical variation (such as close to the main trunk of the portal vein, and it is difficult to safely separate the single-port viewing angle); 4. Ethical exclusion: not approved by the hospital ethics committee or refusal of the guardian to participate in the study. |
