研究目的：

基于工程化丁酸梭菌递送GLP-1的临床前研究数据，验证其治疗肥胖的有效性（体重下降、代谢改善）和安全性（胃肠道反应、系统毒性）。  

Objectives of Study：

Preclinical study data based on engineered Clostridium butyricum for GLP-1 delivery to verify its efficacy in treating obesity (weight loss, metabolic improvement) and safety (gastrointestinal reactions, systemic toxicity).

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄与BMI：18-65岁，BMI>=30 kg/m^2（肥胖）或BMI>=27 kg/m62伴至少一项肥胖相关并发症（如高血压、2型糖尿病、非酒精性脂肪肝）。 2.代谢稳定性：空腹血糖<=13.9 mmol/L，糖化血红蛋白（HbA1c）<=9.0%（若合并糖尿病）。 3.治疗史：筛选前3个月内未接受GLP-1受体激动剂、胰岛素或减重手术干预。 4.肠道菌群状态：无近期（6个月内）抗生素或益生菌使用史，肠道菌群多样性指数（Shannon指数）>=3.0。

Inclusion criteria

1. Age and BMI: 18-65 years old, BMI >=30 kg/m^2 (obesity) or BMI>=27 kg/m62 with at least one obesity-related complication (such as hypertension, type 2 diabetes, non-alcoholic fatty liver disease). 2. Metabolic stability: fasting blood glucose <=13.9 mmol/L, glycosylated hemoglobin (HbA1c) <=9.0% (if combined with diabetes). 3. Treatment history: No GLP-1 receptor agonist, insulin, or bariatric surgical intervention within 3 months prior to screening. 4. Intestinal microbiota status: no recent (within 6 months) history of antibiotic or probiotic use, and the intestinal microbiota diversity index (Shannon index) >=3.0.

排除标准：

1.合并疾病：严重肝肾功能不全（ALT/AST>3倍上限，eGFR<30 mL/min/1.73m^2）、炎症性肠病、免疫缺陷或活动性肿瘤。 2.药物干扰：长期使用糖皮质激素、免疫抑制剂或影响胃肠动力药物（如奥利司他）。 3.妊娠与哺乳：育龄期女性需采取有效避孕措施，妊娠试验阴性。 4.过敏史：对大肠杆菌或工程菌载体成分过敏

Exclusion criteria：

1. Comorbid diseases: severe liver and kidney insufficiency (ALT/AST> 3 times the upper limit, eGFR<30 mL/min/1.73m^2), inflammatory bowel disease, immunodeficiency or active tumor. 2. Drug interference: long-term use of glucocorticoids, immunosuppressants, or drugs that affect gastrointestinal motility (such as orlistat). 3. Pregnancy and breastfeeding: Women of childbearing age need to take effective contraceptive measures and have a negative pregnancy test. 4. History of allergies: allergy to E. coli or engineered bacterial carrier components
