探索在现有的慢性心力衰竭随访计划中增加线上远程随访的模式和效果；慢性心衰患者院外健康管理的智能化发展  

Objectives of Study：

Explore the mode and effectiveness of adding online remote follow-up to existing chronic heart failure follow-up plans; Intelligent development of outpatient health management for patients with chronic heart failure

药物成份或治疗方案详述：

纳入标准：

针对病情稳定但需持续监测的慢性心衰患者 （1）年龄大于18岁的患者； （2）能够使用智能手机的患者； （3）需确诊为心力衰竭。

Inclusion criteria

For chronic heart failure patients with stable condition but requiring continuous monitoring (1) Patients over 18 years old; (2) Patients who are able to use smartphones; (3) Diagnosed with heart failure.

排除标准：

（1）有先天性心脏病的患者； （2）有认知障碍或精神类疾病的患者； （3）正在参加其他相关研究的患者； （4）患有已知严重的进展性疾病（如恶性肿瘤、肝功能不全或肝病）或使患者极度衰竭的疾病，预计生存时间＜12个月; （5）孕妇或拟怀孕者; （6）任何可能干扰研究进程的情况，例如痴呆、瘫痪、酗酒等.

Exclusion criteria：

(1) Patients with congenital heart disease; (2) Patients with cognitive impairments or mental illnesses; (3) Patients who are currently participating in other related studies; (4) Suffering from known severe progressive diseases (such as malignant tumors, liver dysfunction, or liver disease) or diseases that cause extreme exhaustion in patients, with an estimated survival time of less than 12 months; (5) Pregnant or prospective pregnant women; (6) Any situation that may interfere with the research process, such as dementia, paralysis, alcohol abuse, etc.

1.在出院时，医疗团队工作人员对常规随访组的参与者进行健康宣教和指导。 2.出院以后，医疗团队工作人员对患者进行常规随访，即在患者出院后1个月、3个月、6个月时对他们进行电话随访，在电话随访中主要是解他们的疾病自我监测情况以及居家服药、居家饮食、身体锻炼等疾病自我管理的情况，并进行健康宣教和提醒他们及时复诊。

1.At discharge, medical team staff provide health education and guidance to participants in the routine follow-up group. 2. After discharge, the medical team conducts routine follow-up on patients, that is, telephone follow-up at 1 month, 3 months, and 6 months after discharge. During the telephone follow-up, the main purpose is to understand their self-monitoring of diseases and self-management of diseases such as home medication, home diet, and physical exercise, and to provide health education and remind them to follow up in a timely manner.
