研究目的：

经口内镜下食管肌切开术（POEM）已被认为是治疗贲门失弛缓症及其他食管动力障碍的一种安全有效的微创手术方法。尽管POEM是微创内镜侵入性明显低于腹腔镜海勒肌切开术，但胸骨后疼痛和反流在术后仍然常见。术后的疼痛源自食管和胃部，食管受迷走神经和脊髓传入纤维的双重支配，兼具躯体和内脏疼痛的成分。有报告称，经口内镜下肌切开术（POEM）后胸痛的发生率高达 67%，重度疼痛的发生率更是高达10%，这严重影响了患者的快速康复和术后的生活质量。不仅会影响患者的进食，还包括相关的不愉快或令人厌恶的情感体验，并增加术后并发症的发生几率和住院时间。如何预防并控制POEM全麻术后疼痛成为亟需解决的热点问题。影响POEM术后疼痛因素较多，但防治措施效果欠佳。本研究通过多因素logistic回归分析方法，分析POEM全麻术后疼痛相关因素并探讨其危险因素，以便为防治措施发生提供临床参考。  

Objectives of Study：

Peroral endoscopic esophageal myotomy (POEM) has been established as a safe and effective minimally invasive surgical approach for treating achalasia and other esophageal motility disorders. Although POEM is a minimally invasive endoscopic procedure with significantly lower invasiveness than laparoscopic Heller myotomy, retrosternal pain and reflux remain common postoperative complications. Postoperative pain originates from the esophagus and stomach, where the esophagus is dually innervated by the vagus nerve and spinal afferent fibers, contributing to both somatic and visceral pain components. Reports indicate that the incidence of chest pain following POEM reaches as high as 67%, with severe pain affecting 10% of patients. This significantly hinders rapid postoperative recovery and impairs quality of life, compromising dietary intake, inducing unpleasant or distressing emotional experiences, and increasing the risk of postoperative complications and hospital stay duration. The prevention and management of postoperative pain after POEM under general anesthesia have emerged as critical clinical challenges. Despite numerous identified factors influencing POEM postoperative pain, current preventive and therapeutic strategies have shown suboptimal efficacy. This study employs multivariate logistic regression analysis to identify correlates of postoperative pain after POEM under general anesthesia and characterize its risk factors, aiming to provide clinical insights for the development of targeted preventive and management strategies.

纳入标准：

1.无性别限制，年龄18至80岁； 2.美国麻醉医师协会（ASA）身体状况分级为I–II级； 3.临床诊断为贲门失弛缓症； 4.具备完整的麻醉记录和手术报告。

Inclusion criteria

1. No gender restrictions, age 18 to 80 years old; 2. American Society of Anesthesiologists (ASA) physical condition classification I–II; 3. Clinical diagnosis of achalasia; 4. Have complete anesthesia records and surgical reports.

排除标准：

1.存在食管胃底静脉曲张、活动性胃肠道恶性肿瘤、妊娠、对吗啡等阿片类药物过敏； 2.术中出现影响术后疼痛评估的并发症，包括但不限于食管穿孔、皮下气肿、气胸、纵隔气肿、气腹、胸腔积液等。

Exclusion criteria：

1. Presence of esophageal and gastric varices, active gastrointestinal malignancy, pregnancy, allergy to opioids such as morphine; 2. Complications that affect postoperative pain assessment, including but not limited to esophageal perforation, subcutaneous emphysema, pneumothorax, pneumomediastinum, pneumoperitoneum, pleural effusion, etc.
