研究目的：

本研究旨在通过一项基于社区的随机对照试验，比较动态血糖监测（CGM）与居家指尖血血糖监测（SMBG）对2型糖尿病患者自我管理效果的影响。  

Objectives of Study：

This community-based randomized controlled trial aims to compare the effects of continuous glucose monitoring (CGM) versus self-monitoring of blood glucose (SMBG) on self-management outcomes in patients with type 2 diabetes.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、确诊2型糖尿病患者,半年内糖化血红蛋白（HbA1c）≥8%； 2、已开始服用口服降糖药，暂未使用胰岛素； 3、具有完全认知和行为能力，有正常读写能力,会用智能手机; 4、愿意在一个月内参加社区体检，完成体格检查、空腹血糖、血脂四项、b超、心电图、心音心电监测、血常规、尿常规、糖化血红蛋白（HbA1c）； 5、没有使用过动态血糖监测仪器； 6、同意剩下的生物样本用于后续研究。

Inclusion criteria

1.Confirmed diagnosis of type 2 diabetes mellitus (T2DM), HbA1c ≥8% within the past 6 months; 2.Currently on oral hypoglycemic agents (OHA),No prior insulin therapy; 3.Fully intact cognitive and behavioral abilities and Literate with smartphone proficiency; 4.Willing to complete community-based examinations within 1 month; 5.No prior use of continuous glucose monitoring (CGM) systems; 6.Agree to preserve residual biological samples for future research

排除标准：

1、1型糖尿病或其他类型糖尿病患者； 2、糖尿病前期患者； 3、半年内糖化血红蛋白（HbA1c）<8%； 4、近期低血糖昏迷史； 5、长期使用动态血糖监测仪器。

Exclusion criteria：

1.Type 1 diabetes or other specific diabetes types (e.g., MODY, LADA); 2.Prediabetes (IFG/IGT); 3.HbA1c <8% in the past 6 months, 4.History of hypoglycemic coma within 3 months; 5.Long-term CGM users (>30 days cumulative).
