研究目的：

主要研究目的:通过随机对照研究，评价 5°和 15°角度俯卧位通气方式对机械通气患者氧合指数、血流动力学的影响。 次要研究目的通过随机对照研究，评价 5°和 15°角度俯卧位通气方式对机械通气患者压力性损伤发生情况、气道相关并发症发生率、机械通气时间、ICU 住院天数、护理服务工作满意度的影响。  

Objectives of Study：

Primary study objective:To evaluate the effect of 5° and 15° angle prone ventilation on oxygenation index and hemodynamics of mechanically ventilated patients in a randomized controlled study. Secondary Research Objective:To evaluate the effect of 5° and 15° angle prone position ventilation on the occurrence of pressure injury, airway-related complications, duration of mechanical ventilation, ICU stay, and satisfaction with nursing services in mechanically ventilated patients through a randomized controlled study.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄＞＝18 岁；2.符合俯卧位机械通气指征，每次俯卧位时间＞＝12h；3.入 ICU 时已行俯卧位机械通气＞＝24h；4.预计俯卧位机械通气时间＞＝48h 的患者；5.征得患者本人或家属同意自愿参加本研究。

Inclusion criteria

The patients were 1. aged >=18 years; 2. met the indications for prone mechanical ventilation, and the duration of each prone position was >=12h; 3.had already been on prone mechanical ventilation for >=24h at the time of admission to the ICU; 4. the expected duration of prone mechanical ventilation was >=48h; and 5. obtained the consent of the patients or their family members to participate in this study voluntarily.

排除标准：

1.存在俯卧位禁忌症，包括面/颈部外伤或脊柱未固定者；近期行开胸或开腹手术；大面积烧伤；颅内压升高；咯血；需行心肺复苏或除颤的高危患者及医嘱禁忌等；2.患者不配合治疗或不依从体位护理者；3.病情资料及观察指标收集不全影响分析者4.同时参与其他类似研究的患者。

Exclusion criteria：

1.There are contraindications to prone position, including face/neck trauma or unfixed spine; recent open thoracic or open abdominal surgery; large-area burns; elevated intracranial pressure; hemoptysis; high-risk patients requiring cardiopulmonary resuscitation or defibrillation, and contraindications to medical prescriptions, etc.; 2.patients who do not cooperate with the treatment or do not comply with the positional care; 3.the collection of condition data and observation indexes is incomplete and affects the analysis of the patients; 4.the patients who are also participating in other similar studies.

仰卧位时，将床头抬高 30°，并于患者骶尾部贴减压贴预防压力性损伤。俯卧位时床面与地面倾斜 5°，其余措施均采用常规护理。

In the supine position, the head of the bed was elevated by 30°, and the patient's sacrococcygeal area was plastered with pressure-relieving patches to prevent pressure injuries. In the prone position, the bed was tilted 5° to the floor, and the rest of the measures were adopted as routine care.

仰卧位时，将床头抬高 30°；俯卧位时，将床面倾斜15°，使患者处于头高脚低位，使用软枕置于患者双侧肩部、髋部两侧，预防患者移位或产生移位倾向；并于床尾置软枕，防止患者因重力下滑或产生摩擦力，其余措施均同常规组。

In supine position, the head of the bed was elevated by 30°; in prone position, the bed was tilted by 15°, so that the patients were in the head-high-feet-low position, and soft pillows were placed on both sides of the patient's bilateral shoulders and hips, to prevent the patient from shifting or generating a tendency of shifting; and soft pillows were placed on the end of the bed, to prevent the patient from sliding down or generating friction due to gravity, and the rest of measures were the same as in the conventional group.

本研究采用便利抽样、随机分组方法。便利抽样的群体是 2025 年 7 月-2025年 9 月入住广东省某三级甲等综合医院内科 ICU、外科 ICU 符合纳排标准的机械通气患者，为避免沾染，在两个 ICU 中分别设立两组：使用 SPSS26.0 程序生成的随机数字表将研究对象分为俯卧位 5°组及俯卧位 15°组，每组各 24 例。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Convenience sampling and randomized grouping methods were used in this study. The convenience sampling group was mechanically ventilated patients who were admitted to the medical ICU and surgical ICU of a tertiary general hospital in Guangdong Province from July 2025 to September 2025 and met the criteria for natriuresis, and to avoid contamination, two groups were set up in each of the two ICUs: the study participants were divided into the prone 5° group and the prone 15° group using a table of randomized numbers generated by the SPSS26.0 program, with 24 cases in each group. Each group consisted of 24 cases.

盲法：

鉴于行为干预的特点，对干预人员和研究对象无法实施盲法。但是本研究中的干预后的数据收集者将不被告知分组情况。同时，医院的工作人员也不被告知分组情况，而研究对象则被要求不允许告知其他人自己的分组情况。从而，本研究可以实现对干预后的数据收集者实施盲法。

Blinding：

Given the nature of behavioral interventions, blinding of interventionists and study participants is not possible. However, the post-intervention data collectors in this study will not be informed of the grouping. Also, hospital staff will not be informed of the grouping, and study participants will be asked not to be allowed to inform others of their grouping. Thus, this study can achieve the implementation of blinding for the post-intervention data collectors.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1 病例报告表/电子数据记录 研究者查阅患者在基线，干预期间和干预后 24h 内的病历记录，所有数据将录入 SPSS27.0 软件进行保管。 2 数据管理 采用双人录入的方法录入数据，对文档进行加密保护。
