研究目的:

本研究通过整合多模态磁共振成像数据,动态评估 PFO 伴偏头痛患者在 PFO 封堵术前后的脑网络变化,揭示 PFO 相关偏头痛的神经网络重塑过程及其潜在机制。研究将重点关注脑功能连接、局部神经活动以及大脑功能网络的变化,以探索 PFO 封堵术对大脑神经可塑性的影响。本研究不仅可为深入理解 PFO 相关偏头痛的病理生理机制提供重要线索,还可为优化术后干预策略提供神经影像学依据,通过针对大脑特定神经反应模式的个性化治疗,提高 PFO 封堵术的临床疗效,减少术后偏头痛的发作频率及严重程度,从而改善患者的整体生活质量。  

Objectives of Study:

This study integrates multimodal magnetic resonance imaging data to dynamically assess brain network changes in patients with PFO-related migraine before and after PFO closure, revealing the neural network remodeling process and its potential mechanisms. The study will focus on alterations in brain functional connectivity, local neural activity, and large-scale brain functional networks to explore the impact of PFO closure on brain neuroplasticity. This research not only provides critical insights into the pathophysiological mechanisms of PFO-related migraine but also offers neuroimaging evidence for optimizing postoperative intervention strategies. By enabling personalized treatments targeting specific neural response patterns in the brain, this study aims to enhance the clinical efficacy of PFO closure, reduce the frequency and severity of postoperative migraine attacks, and ultimately improve patients' overall quality of life.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.经国际头痛疾病分类第三版(ICHD-III)诊断为偏头痛; 2.经对比增强经颅多普勒超声诊断为卵圆孔未闭(PFO); 3.年龄在18-60岁之间,男女不限; 4.计划接受PFO封堵术的患者; 5.能够配合完成术前、术后随访影像学检查;无严重的神经系统疾病或精神疾病史; 6.签署知情同意书。

Inclusion criteria

1. Diagnosed with migraine by the International Classification of Headache Disorders, Third Edition (ICHD-III); 2. Patent foramen ovale (PFO) diagnosed by contrast-enhanced transcranial Doppler ultrasound; 3. Age between 18-60 years old, male or female; 4. Patients who are scheduled to undergo PFO occlusion; 5. Able to cooperate with the completion of preoperative and postoperative follow-up imaging examinations; No history of severe neurological disease or psychiatric illness; 6. Sign the informed consent form.

排除标准:

1.图像质量不佳; 2.存在磁共振扫描禁忌症,例如金属植入物,幽闭恐惧症等; 3.既往存在发育迟缓、药物过度使用、癫痫、脑卒中、脑出血; 4.妊娠期或哺乳期妇女;颅内器质性病变及精神类疾病。

Exclusion criteria:

1. Poor image quality; 2. Presence of contraindications to magnetic resonance scanning, such as metal implants, claustrophobia, etc.; 3. Previous developmental delay, drug overuse, epilepsy, stroke, and cerebral hemorrhage; 4. Pregnant or lactating women; Intracranial organic lesions and psychiatric diseases.
