研究目的:

本研究致力于评估功能恢复模型在膝前交叉韧带(ACL)重建术后康复中的应用效果,特别是针对高运动需求人群。随着全民健身趋势的推进,运动相关伤害日益增多,其中ACL损伤尤为突出,成为公共健康关注的重点。在此背景下,ACL重建术后的优化康复方案显得尤为重要,直接关系到患者能否安全、高效地返回运动领域。  

Objectives of Study:

This study is dedicated to evaluating the effectiveness of functional recovery modeling in postoperative rehabilitation of anterior cruciate ligament(ACL) rrconstrucation of the knee,especially for the high sports demand population. With the advancement of the trend of national fitness,sports-related injuries are increasing,among which ACL injuries are particularly prominent and hace become a public health concern.In this context,optimized rehabilitation protocols after ACL reconstruction are particularly important and directly related to whether patients can return to sports safely and efficiently.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.高运动需求者:每周积极参与运动项目 3 次以上,Tegner 评分大于 5 分(Klasan, A.2021),受伤前参加过 I 级运动(即涉及切削和旋转类活动)(NOYES, F. R. 1989); 2.主要诊断为前交叉韧带损伤,接受过自体骨-髌腱-骨(BPTB)或腘绳肌(HS)移植的前交叉韧带复位术。 3.手术时年龄在 18-45 岁之间; 4.无认知障碍,能够配合康复训练和随访。

Inclusion criteria

1. High exercise demand: active participation in sports programs more than 3 times per week, Tegner score greater than 5 (Klasan, A.2021), participation in Class I sports (i.e., activities involving cutting and rotation) prior to injury (NOYES, F. R. 1989); 2. Anterior cruciate ligament reduction with a primary diagnosis of anterior cruciate ligament injury who has undergone autologous bone-patellar tendon-bone (BPTB) or hamstring (HS) transplantation. 3. Be between 18-45 years of age at the time of surgery; 4. No cognitive impairment, able to cooperate with rehabilitation training and follow-up.

排除标准:

1. 使用同种异体或合成移植物进行 ACLR; 2.前交叉韧带翻修手术; 3. 根据IKDC分类,其他韧带损伤等级为C级或D级; 4.在进行 ACLR 的同时进行了半月板修复手术; 5.根据ICRS分类,软骨损伤等级为III级或IV级。

Exclusion criteria:

1. ACLR with allogeneic or synthetic grafts; 2. Anterior cruciate ligament revision surgery; 3. Other ligament injuries are classified as C or D according to the IKDC classification; 4. Meniscus repair surgery at the same time as ACLR; 5. Cartilage damage grade III or IV according to the ICRS classification.

1.功能性康复方案 第一阶段(1-4周) 目标:保护植入物,减轻关节肿胀和疼痛,恢复髌股关节活动度和正常膝关节被动伸展功能,重建股四头肌自主控制能力,实现独立行走。 训练内容:下肢抬高,踝泵动作,四向髌骨活动,下肢负重,恢复被动伸展的练习,早期强化,平衡和本体感觉练习等。 注意事项:消炎,止痛,冰敷控制肿胀,早期使用CPM,Kinesio-taping减轻疼痛和肿胀。 第二阶段(4-6周) 目标:逐渐增加膝关节屈曲,直腿抬高时膝关节不能伸直,不再使用支具,不再使用拐杖,开始肌肉、本体感觉和平衡训练。 训练内容:有限运动范围的开放式动力链练习,继续第一阶段的脚跟滑动、股四头肌/腘绳肌和直腿抬高练习,站立姿势完成提踵练习,闭链膝关节伸展练习等。 第三阶段 目标与训练计划包括膝关节伸直抗阻训练,恢复股四头肌和腘绳肌力量,闭链运动强度提高,小腿力量强化,臀肌、腰-骨盆-髋核心训练,本体感觉和平衡觉提升等。 第四阶段 目标:进一步增强力量、爆发力和力量,改善运动技术手势,身体调节,运动专项动作和技能训练等。 训练计划:包括以直线为基础的跑步,基础运动任务,减速任务,移动能力练习,高负荷器械强化训练等。 第五阶段 目标:继续恢复肌肉力量失衡,核心稳定性,功能性力量,运动模式化,减速/着陆能力,单向协调性等。

干预措施代码:

Intervention:

1.Functional Rehabilitation Program Phase I (1-4 weeks) Objectives: to protect the implant, to reduce joint swelling and pain, to restore patellofemoral joint mobility and normal knee passive extension, to rebuild the quadriceps muscle autonomy control ability, to realize independent walking. Training content: lower limb elevation, ankle pump action, four-way patellar activity, lower limb weight bearing, exercises to restore passive extension, early strengthening, balance and proprioception exercises, etc. Precautions: anti-inflammatory, analgesic, ice to control swelling, early use of CPM, Kinesio-taping to reduce pain and swelling. Phase II (4-6 weeks) Objectives: Gradual increase of knee flexion, no knee extension during straight leg raise, no more braces, no more crutches, start muscle, proprioceptive and balance training. Training: open power chain exercises with limited range of motion, continue Phase I heel slide, quadriceps/hamstrings and straight leg raise exercises, complete heel raises in standing position, closed chain knee extension exercises, etc. Phase III Objectives and training program include knee extension resistance training, restoration of quadriceps and hamstring strength, closed chain exercise intensity increase, calf strength strengthening, gluteal, lumbar-pelvic-hip core training, proprioception and balance enhancement. Stage 4 Objectives: further increase strength, explosiveness and power, improve sports technical gestures, body conditioning, sports-specific movement and skill training, etc. Training program: including running based on straight line, basic movement tasks, deceleration tasks, mobility exercises, high load equipment strengthening training, etc. Phase 5 Goal: Continue to restore muscular strength imbalances, core stability, functional strength, movement patterning, deceleration/landing ability, unidirectional coordination, etc.

传统康复方案详细分阶段 第一阶段(0-2周)目标:增加膝关节ROM,增加股四头肌力量。治疗措施:佩裁关节保护性支具、踝泵运动、髌骨松动、关节活动度训练、直腿拾高,冰敷、调制中频电治疗,被动膝关节屈曲训练等。第二阶段(3-6周) 目标:控制肿胀和疼痛,提高髌骨活动度,增强下肢力量强度和肌耐力,增强神经肌肉控制。治疗措施:继续第一阶段的踝泵运动、股四头肌等长收缩训练,直腿拾高,佩戴膝关节支具下进行可耐受的负重训练、双脚站立位提踵等。第三阶段(6-12周) 目标:恢复正常ROM,进一步提高下肢肌肉强度、肌力、肌耐力、提高下肢灵活性。治疗措施:继续膝关节ROM练习,步态练习强化下肢肌力,靠墙静蹲,上下台阶练习,练习快走、功率自行车等。第四阶段(12-20周) 目标:主动及被动角度恢复正常,提高髋关节和膝关节周围肌肉力量,增加下肢灵活性,改善平衡和协调能力,恢复本体感觉,增强信心。治疗措施:单侧平衡本体感觉训练,户外慢跑,肌肉力量增强,俯卧勾腿,单腿提踵,弹力带终末伸膝/屈膝练习等。第五阶段(20周以上) 目标:对专项运动动作没有恐惧感,获得最大力量和灵活性,满足专项运动的要求。

Intervention:

Traditional Rehabilitation Program Detailed Phases Phase I (0-2 weeks) Objectives:Increase knee ROM, increase quadriceps strength. Treatment measures: wear joint protective braces, ankle pump exercise, patella release, joint mobility training, straight leg pick up, ice, modulated IF electrotherapy, passive knee flexion training. Phase II (3-6 weeks) Objectives: Control swelling and pain, improve patellar mobility, enhance lower limb strength and endurance, enhance neuromuscular control. Therapeutic measures: continue the ankle pumping exercise, quadriceps isometric contraction training, straight leg pick up, wear knee support under the tolerable weight-bearing training, two-legged standing position heel lift. Phase III (6-12 weeks) Objective: To restore normal ROM, further improve muscle strength, muscle power, muscle endurance and flexibility of the lower limbs. Therapeutic measures: continue knee ROM exercises, gait exercises to strengthen lower limb muscle strength, squatting against the wall, up and down steps, walking fast, power cycling, etc. Phase 4 (12-20 weeks) Objectives: Normalization of active and passive angles, improvement of muscle strength around the hip and knee joints, increase in flexibility of the lower limbs, improvement of balance and coordination, restoration of proprioception, and enhancement of confidence. Treatment measures: unilateral balance proprioception training, outdoor jogging, muscle strength enhancement, prone leg hooking, single-leg heel lifting, elastic band terminal knee extension/flexion exercises, etc. Stage 5 (20 weeks or more) Objectives: No fear of specialized sports movements, maximum strength and flexibility to meet the requirements of specialized sports.
