研究目的：

本研究旨在证明20Gy术前放疗剂量可以更好地平衡肿瘤缩小与受照野血管状态的关系，既对术前抗肿瘤治疗有效，也能提供更好的皮瓣移植环境，提高皮瓣移植修复的存活率。同时，研究将探讨20Gy小剂量放疗对免疫微环境的具体影响，包括PD-L1表达与新辅助治疗效果的相关性，以及放疗对免疫细胞功能和肿瘤微环境的影响。  

Objectives of Study：

This study aims to prove that the 20Gy preoperative radiotherapy dose can better balance the relationship between tumor shrinkage and the vascular status of the irradiated field. It is not only effective for preoperative anti-tumor treatment, but also provides a better environment for skin flap transplantation and improves skin flap transplantation. Repair survival rate. At the same time, the study will explore the specific impact of 20Gy low-dose radiotherapy on the immune microenvironment, including the correlation between PD-L1 expression and the effect of neoadjuvant therapy, as well as the impact of radiotherapy on immune cell function and the tumor microenvironment.

药物成份或治疗方案详述：

患者分组：试验组术前放疗组接受20Gy放疗后进行手术，术后3-4周再进行30-40Gy放疗。对照组直接手术, 术后3-4周再进行50-60Gy放疗。 1) 标本收集：术中收集癌及癌旁组织标本，固定液保存。 2) 免疫组化染色分析：评估血管状态，比较不同剂量放疗对血管的影响。 3) 单细胞测序：分析放疗前后免疫细胞的变化。 4) 使用OHIP-14口腔健康影响程度量表评估患者生活质量。  

Description for medicine or protocol of treatment in detail：

Patient grouping: The trial group received 20Gy preoperative radiotherapy before surgery, and then received 30-40Gy radiotherapy 3-4 weeks after surgery. The control group underwent direct surgery and then received 30-40Gy radiotherapy 3-4 weeks after surgery. 1) Specimen collection: Collect cancer and adjacent tissue specimens during surgery and preserve them in fixative. 2) Immunohistochemical staining analysis: evaluate the status of blood vessels and compare the effects of different doses of radiotherapy on blood vessels. 3) Single cell sequencing: Analyze changes in immune cells before and after radiotherapy. 4) Assessing patient quality of life using the OHIP-14 Oral Health Impact Scale. 

纳入标准：

纳入标准：1.经病理科诊断为口腔鳞状细胞癌，且根据第八版美国癌症联合委员会(AJCC)分期手册进行临床分期为Ⅳ期的患者；2.原发口腔癌切除后必须同时进行血管化游离组织瓣修复的患者；3.无远处转移；4.无有肺、肝远部位肿瘤转移患者；5.同意手术治疗的患者。

Inclusion criteria

Inclusion criteria: 1. Patients diagnosed with oral squamous cell carcinoma by the pathology department and clinically staged as stage IV according to the eighth edition of the American Joint Committee on Cancer (AJCC) staging manual; 2. The primary oral cancer must be resected at the same time. Patients with vascularized free tissue flap repair; 3. No distant metastasis; 4. No patients with distant lung or liver tumor metastasis; 5. Patients who agree to surgical treatment.

排除标准：

排除标准：1.患者存在严重系统疾病身体无法耐受手术患者；2.有肺、肝远部位肿瘤转移患者；3.不同意手术治疗及术后放疗的患者；3.其他种类的口腔颌面部恶性肿瘤非鳞状细胞癌患者；4.已接受术前放疗和化疗的患者。

Exclusion criteria：

Exclusion criteria: 1. Patients with severe systemic diseases who cannot tolerate surgery; 2. Patients with tumor metastasis in distant parts of the lungs and liver; 3. Patients who do not agree to surgical treatment and postoperative radiotherapy; 3. Other types of oral and maxillofacial diseases Patients with local malignant tumors other than squamous cell carcinoma; 4. Patients who have received preoperative radiotherapy and chemotherapy.
